摘  要:目的:探讨清胰解毒大黄汤通过丝裂原活化蛋白激酶(MAPK)通路治疗小鼠急性重症胰腺炎的机制。方法:60只小鼠随机分为4组(n=15),分别为对照组、模型组、干预组、抑制剂组。腹腔注射雨蛙肽溶液建立急性重症胰腺炎模型,干预组使用清胰解毒大黄汤灌胃,抑制剂组腹腔注射SB202190。检测各组胰腺湿干比和HE染色结果。ELISA用于检测血清可溶性细胞间黏附分子-1(sICAM-1)及肿瘤坏死因子(TNF-α)水平。Western blot检测胰腺组织中p38MAPK、sICAM-1及TNF-α蛋白水平。结果:模型组小鼠胰腺湿干比和血清淀粉酶显著高于对照组(P<0.01),干预组和抑制剂组的胰腺湿干比和血清淀粉酶显著低于模型组(P<0.01)。HE染色结果显示模型组细胞质液泡减少,出现明显的炎症浸润减轻和薄壁组织坏死;干预组和抑制剂组的炎症浸润和损伤均减轻。模型组小鼠血清sICAM-1和TNF-α的水平显著高于对照组(P<0.01),干预组和抑制剂组的血清sICAM-1和TNF-α的水平显著低于模型组(P<0.01)。模型组的小鼠胰腺组织p38MAPK、sICAM-1及TNF-α蛋白水平显著高于对照组(P<0.01),干预组和抑制剂组p38MAPK、sICAM-1及TNF-α蛋白水平显著低于模型组(P<0.01)。结论:清胰解毒大黄汤可能通过抑制p38MAPK通路,下调sICAM-1和TNF-α的表达,减少炎症反应和组织损伤,治疗急性重症胰腺炎。Objective:To explore the mechanism of qingyi jiedu dahuang decoction in treating acute severe pancreatitis in mice through mitogen-activated protein kinase(MAPK)pathway.Methods:Sixty mice were randomly divided into 4 groups(n=15),which were control group,model group,intervention group and inhibitor group.The model of acute severe pancreatitis was established by intraperitoneal injection of cerulein solution.The intervention group was treated with qingyi jiedu dahuang decoction and the inhibitor group was injected intraperitoneally with SB202190.The pancreas wet-to-dry ratio and HE staining results of each group were measured.ELISA was used to detect serum soluble intercellular adhesion molecule-1(sICAM-1)and tumor necrosis factor(TNF-α)levels.Western blot was used to detect p38MAPK,sICAM-1 and TNF-αprotein levels in pancreatic tissue.Results:The pancreas wet-dry ratio and serum amylase in the model group were significantly higher than those in the control group(P<0.01).Pancreatic wet-to-dry ratio and serum amylase were significantly lower in the intervention group and the inhibitor group than in the model group(P<0.01).HE staining results showed that the cytoplasmic vacuoles of the model group decreased,and obvious inflammatory infiltration and thin-wall tissue necrosis occurred.Inflammatory infiltration and injury were all attenuated in both the intervention and inhibitor groups.The levels of serum sICAM-1 and TNF-αin the model group were significantly higher than those in the control group(P<0.01).The levels of serum sICAM-1 and TNF-αin the intervention group and the inhibitor group were significantly lower than those in the model group(P<0.01).The p38MAPK,sICAM-1 and TNF-αprotein levels in the pancreas of the model group were significantly higher than those in the control group(P<0.01).The levels of p38MAPK,sICAM-1 and TNF-αprotein in the intervention group and the inhibitor group were significantly lower than those in the model group(P<0.01).Conclusion:Qingyi jiedu dahuang decoction may treat acute seve
