摘 要:目的:探讨经阴道彩色多普勒超声技术在少见型异位妊娠中的诊断价值。方法:选择2013年1月~2018年12月于我院超声科经阴道彩超初诊和病理证实的705例异位妊娠患者作为研究对象,分为常见型组(522例)和少见型组(183例)。比较两组患者初诊时实验室参数(孕酮P、人绒毛膜促性腺激素)和常规阴道彩超参数(子宫大小、内膜厚度、宫腔囊性结构、盆腔积液),并分析少见型异位妊娠声像图特征。结果:两组在实验室参数及常规阴道彩超参数上差异均无统计学意义(均P>0.05)。经阴道彩超诊断少见型异位妊娠的准确率达97.27%。少见型异位妊娠具有典型和不典型阴道彩超声像图,其中瘢痕妊娠和宫角妊娠可见孕囊样结构,宫颈妊娠和卵巢妊娠可见囊实性包块。结论:实验室参数及常规阴道彩超参数对异位妊娠位置判断的参考价值不大。可根据少见型异位妊娠不同部位的阴道彩超声像特点,提高对少见型异位妊娠诊断的准确率。Objective:To explore the value of transvaginal color Doppler ultrasound indiagnosis of rare ectopic pregnancy.Methods:There were 705 cases of ectopic pregnancy from January 2013 to December 2018 in our hospital who were confirmed by transvaginal ultrasound and pathology,divided into the common type group(522 cases)and the rare type group(183 cases).The laboratory parameters including progesterone and human chorionic gonadotropin,and the conventional vaginal ultrasound parameters containing uterine size,endometrial thickness,the cystic structure of uterine cavity and pelvic effusion were compared between two groups.The sonographic characteristics of rare ectopic pregnancy were analyzed.Results:There were no significant differences in laboratory parameters and conventional vaginal ultrasound parameters between two groups(all P>0.05).The accuracy rate of transvaginal ultrasound in diagnosis of rare ectopic pregnancy was 97.27%.The rare type of ectopic pregnancy had typical and atypical vaginal color ultrasonography.The gestational vesicle structure could be detected in scar pregnancy and uterine horn pregnancy,while cystic solid mass could be founded in cervical pregnancy and ovarian pregnancy.Conclusion:Laboratory parameters and conventional vaginal ultrasound parameters cannot provide effective references for determination of ectopic pregnancy location.The diagnosis accuracy of rare ectopic pregnancy can be improved according to vagina color ultrasound characteristics of the rare ectopic pregnancy in different parts.
