D-二聚体阳性患者在临床科室的分布及相关疾病的分析

摘　　要：目的:研究D-二聚体阳性患者在临床科室的分布、所患疾病比例及临床意义。方法:选取本院采用胶乳免疫比浊法测定血浆D-二聚体水平的632例患者,统计阳性患者的各科室分布,并进一步分析所患疾病种类及比例。结果:632例患者中,有316例D-二聚体阳性,总阳性率为50%,其中急诊与创伤外科、骨科、血管外科、妇科、风湿免疫科、儿科及血液内科阳性率均在50%以上。D-二聚体阳性主要来自于炎症、恶性肿瘤、骨折、自身免疫性疾病患者,其次为肢体深静脉血栓及组织损伤患者,此外为冠心病、脑梗死、淋巴瘤、肺栓塞、白血病及其他疾病患者。结论:血浆D-二聚体水平阳性,除与血栓性疾病密切相关外,还与炎症、恶性肿瘤、自身免疫性疾病等亦有关联。Objective:To study the distribution,disease proportion and clinical value of D-dimer positive patients in clinical departments.Methods:A total of 632 patients admitted to our hospital were enrolled,and the plasma D-dimer level was determined by latex immunoturbidimetry.The distribution of positive patients in each department was counted.The types and proportion of diseases were statistically further analyzed.Results:There were 316 patients performed with D-dimer positive,accounting for 50%of the total.The Ddimer positive rates in department of emergency and trauma surgery,orthopedics,vascular surgery,gynecology,rheumatism immunology,pediatric and hematology were all more than 50%.The patients of Ddimer positive were mainly with inflammation,malignant tumor,fracture,autoimmune diseases,followed by patients with deep vein thrombosis of limbs and tissue damage,and patients with coronary heart disease,cerebral infarction,lymphoma,pulmonary embolism,leukemia and other diseases.Conclusion:The positive level of plasma D-dimer is not only closely related to thrombotic diseases,but also to inflammation,malignancy and autoimmune diseases and so on.
