环磷酰胺结合白芍总苷治疗系统性红斑狼疮的临床疗效和安全性研究

摘　　要：目的研究环磷酰胺结合白芍总苷治疗系统性红斑狼疮的临床疗效和安全性。方法随机将我院2015年2月至2016年8月收治的72例系统性红斑狼疮患者分为两组,单一药物组和药物联合治疗组各36例,两组均使用糖皮质激素,在此基础上,单一药物组予以环磷酰胺治疗,药物联合治疗组结合环磷酰胺与白芍总苷进行治疗,对比两组临床疗效和安全性。结果药物联合治疗组环磷酰胺总用量、日平均糖皮质激素用量较单一药物组明显减少,二者对比差异有显著性意义(P<0.05)。药物联合治疗组SLEDAL评分及尿蛋白较单一药物组明显下降,二者对比差异有显著性意义(P<0.05)。药物联合治疗组尿路感染2例,肺部感染1例,不良反应发生率为8.33%;疾病复发2例,复发率为5.56%。单一药物组尿路感染5例,肺部感染2例,霉菌感染4例,不良反应发生率为30.56%;疾病复发8例,复发率为22.22%。药物联合治疗组不良反应发生率、疾病复发率均低于单一药物组,二者对比差异有显著性意义(χ^2=5.6749、4.1806,P<0.05)。结论结合环磷酰胺与白芍总苷治疗系统性红斑狼疮可在临床疗效和安全性方面取得满意效果。Objective To study the clinical effi cacy and safety of cyclophosphamide combined with total glucosides of paeony in the treatment of systemic lupus erythematosus.Methods 72 patients with systemic lupus erythematosus admitted to our hospital from February 2015 to August 2016 were randomly divided into two groups,36 patients in the single drug group and the drug combination group.The glucocorticoids were used in both groups.On this basis,the single drug group was treated with cyclophosphamide,and the drug combination treatment group was combined with cyclophosphamide and total glucosides of paeony.The clinical effi cacy and safety of the two groups were compared.Results The total dose of cyclophosphamide and the daily average glucocorticoid dosage in the drug combination group were signifi cantly lower than those in the single drug group.There was a signifi cant difference between the two groups(P<0.05).The SLEDAL score and urine protein in the drug combination treatment group were signifi cantly lower than those in the single drug group,and the difference was signifi cant(P<0.05).In the drug combination treatment group,there were 2 cases of urinary tract infection,1 case of pulmonary infection,the incidence of adverse reactions was 8.33%,and 2 cases of disease recurrence,the recurrence rate was 5.56%.There were 5 cases of urinary tract infection in the single drug group,2 cases of pulmonary infection,4 cases of mold infection,the incidence of adverse reactions was 30.56%,and 8 cases of disease recurrence,the recurrence rate was 22.22%.The incidence of adverse reactions and disease recurrence rate in the drug combination treatment group were lower than those in the single drug group,and the difference was significant(χ^2=5.6749,4.1806,P<0.05).Conclusion The combination of cyclophosphamide and total glucosides of paeony can treat systemic lupus erythematosus with satisfactory clinical effi cacy and safety.
