不同切口超声乳化术对糖尿病合并白内障患者眼表的影响

摘　　要：目的探讨不同切口超声乳化术对糖尿病合并白内障患者眼表的影响。方法选择2017年1月至2017年12月收治的糖尿病合并白内障患者70例,随机分为观察组和对照组,每组35例,对照组采用角膜切口,观察组采用角巩膜缘切口,对比两组切口修复时间、眼表疾病指数等的差异。结果①观察组切口修复低于7 d者占34.3%、上皮修复低于7 d者占80.0%显著高于对照组,P<0.05;②观察组手术后1周眼表疾病指数(28.3±5.6)分、手术后3个月眼表疾病指数(28.3±5.6)分显著低于对照组,P<0.05;③观察组泪液分泌量、泪膜破裂时间及角膜染色评分均显著优于对照组,P<0.05。结论角巩膜缘切口治疗糖尿病合并白内障对眼表与泪膜的影响更小,患者恢复更快,值得推广。Objective To investigate the effect of different incision phacoemulsifi cation on ocular surface in diabetic patients with cataract.Method 70 cases diabetic patients with cataract during January 2017~December2017 were collected and divided into the observation group and the control group,35 cases in each group,the control group received corneal incision,the observation group received corneal and scleral incision,the incision repair time,ocular surface disease index were compared.Results(1)In the observation group,34.3%of the incisions were repaired less than 7 days and 80.0%of the epithelium was repaired less than 7 days,which was signifi cantly higher than that in the control group(P<0.05).(2)One week after operation,the ocular surface disease index(OSDI)of observation group was(28.3±5.6).Three months after operation,the OSDI of observation group was(28.3±5.6).The OSDI of observation group was signifi cantly lower than that of control group(P<0.05).(3)The amount of tear secretion,tear fi lm rupture time and corneal staining score in the observation group were signifi cantly better than those in the control group(P<0.05).Conclusion The treatment of diabetes mellitus with cataract by corneoscleral margin incision has less effect on ocular surface and tear fi lm,and patients recover faster,which is worth promoting.
