护士主导型个案管理对胃癌术后患者胃肠功能及自护能力的影响研究

摘　　要：目的探讨护士主导型个案管理对胃癌术后患者胃肠功能和自护能力的影响。方法将94例胃癌手术患者随机分为对照组和试验组,对照组按照胃癌外科护理常规进行护理,试验组在常规护理的基础上,实施护士主导型个案管理。结果试验组患者的首次下床时间、首次排气时间、首次进食时间和术后平均住院日均短于对照组,差异具有统计学意义(p<0.05);患者的自护能力总分及自护责任感、健康知识和自护技能三个维度得分均高于对照组,差异具有统计学意义(p<0.05)。结论护士主导型个案管理能够促进胃癌术后患者的胃肠功能恢复,缩短患者术后平均住院日,提高患者自护能力。Objective To explore the effect of nurse-led case management on gastrointestinal function and self-care ability in postoperative patients with gastric cancer.Methods A total of 94 postoperative patients with gastric cancer were included and randomly divided into the control group(n=47)and the experimental group(n=47).The control group received the routine nursing care,while the experimental group received the nurse-led case management besides the routine nursing care.Results The first ambulation time,the first exhaust time,the first eating time and length of stay after surgery of the experimental group were all shorter than those of the control group(p<0.05).The total score of self-care ability and the score of self-care responsibility,health knowledge and self-care skills were all higher than those of the control group(p<0.05).Conclusion Nurse-led case management can promote the recovery of gastrointestinal function in patients undergoing gastrectomy,reduce the postoperative length of stay,and improve their self-care ability.
