摘　　要：目的探讨心脏瓣膜置换术同期行改良双极房颤射频消融术对患者心功能及心脏结构指标的影响。方法选取2017年1月至2019年1月接受心脏瓣膜置换术治疗的90例患者作为研究对象,根据治疗方法不同将其分为对照组和观察组,各45例。对照组单纯采用心脏瓣膜置换术治疗,观察组在心脏瓣膜置换术治疗基础上同期行改良双极房颤射频消融术。比较两组窦性心律恢复情况、心功能改善情况及心脏结构指标变化情况。结果观察组术后当天、出院时、术后1、3个月的窦性心律恢复率均显著高于对照组(P<0.05);术后6个月,观察组心功能改善总有效率为88.89%,显著高于对照组的60.00%(P<0.05);术后6个月,两组LVEDD、LVESD、IVSD及LVPWD均减小,且观察组小于对照组(P<0.05)。结论心脏瓣膜病患者采用心脏瓣膜置换术同期行改良双极房颤射频消融术能够有效提高预后效果,促进患者心功能及心脏结构指标改善,值得推广。Objective To investigate the effect of modified bipolar atrial fibrillation radiofrequency ablation on cardiac function and cardiac structure in patients undergoing heart valve replacement.Methods From January 2017 to January 2019,90 patients who received heart valve replacement were selected as the study objects.According to the different treatment methods,the patients were divided into control group and observation group,with 45 cases in each group.The control group was treated with heart valve replacement alone,and the observation group was treated with modified portal bipolar atrial fibrillation radiofrequency ablation on the basis of heart valve replacement.The recovery of sinus rhythm,the improvement of cardiac function and the changes in cardiac structural indexes were compared between the two groups.Results The recovery rate of sinus rhythm in the observation group were significantly higher than those in the control group on the day after operation,at the time of discharge,1 and 3 months after operation(P<0.05).At 6 months after operation,the total effective rate of cardiac function improvement in the observation group was 88.89%,which was significantly higher than 60.00%in the control group(P<0.05).At 6 months after operation,LVEDD,LVESD,IVSD and LVPWD in the two groups decreased,and those in the observation group lower than the control group(P<0.05).Conclusion Heart valve replacement with modified bipolar atrial fibrillation radiofrequency ablation in patients with valvular heart disease can effectively improve the prognosis and promote the improvement of heart function and cardiac structure indicators,which is worth promoting.
