摘  要:目的观察自血疗法对寻常型痤疮患者外周血中CD14^+单核细胞TLR2的表达及血清IL-8、TNF-α浓度的影响,初步探讨自血疗法治疗寻常型痤疮的部分抗炎机制。方法选取30例寻常型痤疮患者为治疗组,另在本院招募健康志愿者20例作为对照组。分析治疗组采用自血疗法前、后外周血CD14^+单核细胞TLR2的表达及血清IL-8、TNF-α浓度,并与对照组比较。结果治疗组患者TLR2表达与血清IL-8、TNF-α浓度呈正相关(P<0.05)。治疗组干预前的外周血中CD14^+单核细胞TLR2表达及血清IL-8、TNF-α浓度均高于对照组及干预后(P=0.000)。结论TLR2的表达及血清IL-8、TNF-α浓度与寻常型痤疮的发病相关。自血疗法可通过下调寻常型痤疮患者外周血CD14^+单核细胞TLR2的表达,减少血清IL-8、TNF-α的分泌,从而抑制炎症反应的发生。Objective To observe the effect of self blood therapy on CD14^+ monocyte TLR2 expression in peripheral blood and serum IL-8 and TNF-α concentrations of patients with acne vulgaris,and to explore some anti-inflammatory mechanisms of self blood therapy for acne vulgaris.Methods Thirty patients with acne vulgaris were selected as treatment group,and another 20 healthy volunteers recruited in the hospital were set as control group.The peripheral blood CD14^+ monocyte TLR2 expression and serum IL-8 and TNF-α concentrations in the treatment group before and after self blood herapy were analyzed and compared with the control group.Results There was a positive correlation between TLR2 expression and serum IL-8,TNF-α concentration of patients in the treatment group(P<0.05).The CD14^+ monocyte TLR2 expression in peripheral blood and serum IL-8 and TNF-α concentrations before intervention in the treatment group were higher than those in the control group and after intervention(P=0.000).Conclusion The expression of TLR2 and the serum IL-8 and TNF-α concentrations are related to the pathogenesis of acne vulgaris.Self blood therapy can inhibit the occurrence of inflammatory response by down-regulating the expression of CD14^+ monocyte TLR2 in peripheral blood and reducing the secretion of serum IL-8 and TNF-α in patients with acne vulgaris.
