摘  要:目的探讨骨水泥表面技术联合皮瓣在修复四肢骨与关节感染合并组织缺损中的应用效果。方法2016年4月至2019年3月,我科在骨与关节感染合并组织缺损的治疗过程中,一期彻底清创后,应用抗菌素骨水泥覆盖创面,术后选用敏感抗菌素静脉输注抗感染,后改口服利福平抗感染。二期手术行皮瓣修复组织缺损或骨外露创面病例共30例。观察患者感染复发、术后皮瓣血管危象、皮瓣坏死等并发症发生情况、住院时间及住院费用。结果本研究中患者的平均随访时间(17.4±4.2)个月,平均住院时间(24.2±3.5)d,平均住院费用(20786±5213)元。随访中未发现创面处感染复发,X线片未见骨髓炎病灶形成。结论骨水泥表面技术联合皮瓣是修复骨与关节感染合并组织缺损的有效、安全方法。Objective To explore the application effect of bone cement surface technique combined with flap in repairing infection of bones and joint of the extremities and tissue defect.Methods From April 2016 to March 2019,in the treatment of bone and joint infection combined with tissue defect in our department,after the first stage of debridement,antibiotic bone cement was applied to cover the wound surface,sensitive antibiotic intravenous infusion was used to resistance to infection after treatment,then rifampicin was taken orally instead.There were 30 cases of skin flap repairing tissue defect or bone exposed wound in the second stage operation.The complications of recurrence of infection,postoperative vascular crisis and necrosis of the flap,the hospital stays and hospitalization costs were observed.Results In this study,the average follow-up time of patients was(17.4±4.2)months,the average hospital stays was(24.2±3.5)days,and the average hospitalization costs was(20786±5213)yuan.During the follow-up,no recurrence of infection was found at the wound surface,and no osteomyelitis lesion was observed on the X-ray.Conclusion The combination of bone cement surface technique and flap is an effective and safe method to repair bone and joint infection combined with tissue defect.
