摘 要:目的探讨胰岛素联合营养干预在妊娠期糖尿病患者中的应用效果及对血清25-OH-VitD3、脂肪因子水平的影响。方法选取我院2018年2月至2019年2月收治的82例妊娠期糖尿病患者,按电脑数字表法随机分为对照组(41例,胰岛素)和试验组(41例,胰岛素+营养干预)。比较两组治疗前、后的血糖水平、血清25-OH-VitD3及脂肪因子水平;比较两组的不良妊娠结局发生情况。结果治疗后,两组的空腹血糖、餐后2 h血糖水平均降低,且试验组明显低于对照组(P<0.05)。治疗后,两组的25-OH-VitD3、脂联素(APN)水平均升高,视黄醇结合蛋白4(RBP4)水平均降低,且试验组优于对照组(P<0.05)。试验组不良妊娠结局总发生率明显低于对照组(P<0.05)。结论胰岛素联合营养干预可有效控制妊娠期糖尿病患者血糖水平,并改善血清25-OH-VitD3、脂肪因子水平,进而减少不良妊娠结局的发生。Objective To explore the application effect of insulin combined with nutritional intervention on gestational diabetes and its influences on the levels of serum 25-OH-VitD3 and adipokines.Methods A total of 82 patients with gestational diabetes admitted in our hospital from February 2018 to February 2019 were selected and randomly divided into control group(41 cases,insulin)and experimental group(41 cases,insulin+nutritional intervention)according to the method of computer digital table.The levels of blood glucose,serum 25-OH-VitD3 and adipokines before and after treatment in the two groups were compared;the adverse pregnancy outcomes in the two groups were compared.Results After treatment,the levels of fasting blood glucose and 2 hours postprandial blood glucose in the two groups decreased,and those in the experimental group were significantly lower than the control group(P<0.05).After treatment,the levels of 25-OH-VitD3,adiponectin(APN)in the two groups increased,and retinol binding protein 4(RBP4)decreased,and those in the experimental group were better than the control group(P<0.05).The total incidence of adverse pregnancy outcome in the experimental group was significantly lower than that in the control group(P<0.05).Conclusion Insulin combined with nutritional intervention can effectively control blood glucose levels in patients with gestational diabetes,and improve serum 25-OH-VitD3,adipokines levels,and thus reduce the adverse pregnancy outcomes.
