小剂量罗红霉素联合沙美特罗替卡松治疗老年支气管扩张的临床效果

摘　　要：目的探讨小剂量罗红霉素联合沙美特罗替卡松治疗老年支气管扩张的临床效果。方法选取我院收治的102例老年支气管扩张患者为研究对象,按照随机数表法将其分为对照组与观察组,每组51例。对照组在常规治疗基础上给予小剂量罗红霉素胶囊,观察组在对照组的基础上给予沙美特罗替卡松粉吸入剂。比较两组的临床疗效。结果治疗6个月后,两组患者FEV1、FEV1/FVC、PEF、每日咳痰量、PaO2、PaCO2均显著改善,且观察组优于对照组(P<0.05)。治疗6个月后,两组患者IL-6、TNF-α及MMP-9水平均降低,且观察组低于对照组(P<0.05)。治疗6个月后,两组患者SGRQ各维度评分、胸部CT影像学评分及总分均降低,且观察组低于对照组(P<0.05)。本研究中两组患者均未出现不良反应;观察组治疗总有效率显著高于对照组(P<0.05)。结论小剂量罗红霉素联合沙美特罗替卡松治疗老年支气管扩张具有良好的安全性及有效性,值得临床推广。Objective To investigate the clinical effect of low-dose roxithromycin combined with salmeterol/fluticasone in the treatment of bronchiectasis in the elderly.Methods A total of 102 elderly patients with bronchiectasis admitted in our hospital were selected as the research objects.The patients were divided into control group and observation group according to random number table method,with 51 cases in each group.The control group was given low-dose roxithromycin capsule on the basis of conventional treatment,and the observation group was given salmeterol xinafoate and fluticasone propionate powder for inhalation on the basis of the control group.The clinical effects of the two groups were compared.Results After 6 months of treatment,FEV1,FEV1/FVC,PEF,daily sputum volume,PaO2 and PaCO2 in the two groups improved significantly,and those in the observation group were better than the control group(P<0.05).After 6 months of treatment,the levels of IL-6,TNF-α and MMP-9 in both groups decreased,and those in the observation group were lower than the control group(P<0.05).After 6 months of treatment,the scores of all dimensions of SGRQ,chest CT imaging scores and total scores in both groups decreased,and those in the observation group were lower than the control group(P<0.05).In this study,there were no adverse reactions in the two groups.The total effective rate of treatment in the observation group was significantly higher than that in the control group(P<0.05).Conclusion Low-dose roxithromycin combined with salmeterol/fluticasone in the treatment of bronchiectasis in the elderly has good safety and effectiveness,which is worthy of clinical promotion.
