超声引导下深静脉穿刺置管术在妇产科危重患者中的应用效果

摘　　要：目的探讨超声引导下深静脉穿刺置管术在妇产科危重患者中的应用效果。方法回顾性分析2018年6月29日至2019年4月15日本院收诊的52例妇产科危重患者的临床资料,按照治疗方法的不同将患者分为观察组(超声引导下行深静脉穿刺置管术,26例)与对照组(体表解剖标志定位下行深静脉穿刺置管术,26例)。比较两组患者的导管相关性感染发生率、导管留置天数、一次性穿刺成功率、穿刺用时及不良事件发生情况。结果观察组的导管相关性感染发生率低于对照组,导管留置天数长于对照组,一次性穿刺成功率高于对照组,穿刺用时短于对照组,差异具有统计学意义(P<0.05)。观察组的不良事件总发生率低于对照组,差异具有统计学意义(P<0.05)。结论超声引导下行深静脉穿刺置管术可减少妇产科危重患者的不良事件发生,提高一次性穿刺成功率,值得临床应用推广。Objective To investigate the application effect of ultrasound-guided deep venous catheterization in critical patients of gynecology and obstetrics.Methods The clinical data of 52 critical patients of obstetrics and gynecology admitted in our hospital from June 29,2018 to April 15,2019 were retrospectively analyzed.According to the different treatment methods,the patients were divided into observation group(ultrasound-guided deep venous catheterization,26 cases)and control group(deep venous catheterization under the location of anatomical marks on the body surface,26 cases).The incidences of catheter-related infection,duration of catheter indwelling,success rates of one-time puncture,puncture time and adverse events were compared between the two groups.Results The incidence of catheter-related infection in the observation group was lower than that in the control group,the duration of catheter indwelling was longer than that in the control group,the success rate of one-time puncture was higher than that in the control group,and the puncture time was shorter than that in the control group,the differences were statistically significant(P<0.05).The total incidence of adverse events in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Ultrasound-guided deep venous catheterization can reduce the occurrence of adverse events in critical patients of gynaecology and obstetrics,and increase the success rate of one-time puncture,which is worthy of clinical application and promotion.
