摘　　要：目的探讨经腹与经胸根治术对贲门癌术后患者预后的影响。方法选取我院行贲门癌根治术的72例患者为研究对象,根据手术方式的不同将其分为经胸组和经腹组,各36例。经胸组行经胸贲门癌根治术治疗,经腹组行经腹贲门癌根治术治疗。观察两组手术及术后情况。结果术后两组吻合口瘘、切口感染、腹腔感染发生率无显著差异(P>0.05),但经胸组肺部并发症发生率明显高于经腹组(P<0.05)。经腹组淋巴清扫数目多于经胸组,住院时间短于经胸组(P<0.05)。手术后,两组生活质量及KPS评分均明显提高,且经腹组高于经胸组(P<0.05)。结论经腹贲门癌根治术优于经胸贲门癌根治术,经腹手术淋巴清扫更彻底,术后肺部并发症少,患者的生活质量相对较高,预后较好。Objective To explore the effect of transabdominal and transthoracic radical surgery on the prognosis of patients with cardia cancer after operation.Methods Seventy-two patients who underwent radical surgery of cardia cancer in our hospital were selected as the study objects.According to the different operation methods,the patients were divided into transthoracic group and transabdominal group,with 36 cases in each group.The transthoracic group was treated with transthoracic radical surgery of cardia cancer,and the transabdominal group was treated with transabdominal radical surgery of cardia cancer.The operation and postoperative situation of the two groups were observed.Results There were no significant differences in the incidences of anastomotic leakage,incision infection and abdominal infection between the two groups(P>0.05),but the incidence of pulmonary complications in the transthoracic group was significantly higher than that in the transabdominal group(P<0.05).The number of lymph node dissection in the transabdominal group was more than that in the transthoracic group,and the length of hospitalization was shorter than that in the transthoracic group(P<0.05).After operation,the quality of life and KPS scores in the two groups significantly increased,and those in the transabdominal group were higher than the transthoracic group(P<0.05).Conclusion Transabdominal cardia radical surgery is better than transthoracic cardia radical resection,it has more thorough in lymphadenectomy,less postoperative pulmonary complications,higher quality of life and better prognosis.
