逆行交锁髓内钉治疗股骨下段及髁上骨折的临床效果

摘　　要：目的分析逆行交锁髓内钉治疗股骨下段及髁上骨折的临床效果。方法选择2017年1月至2019年10月在本院接受治疗的58例股骨下段及髁上骨折患者为研究对象,根据患者选择的治疗方法将其分为常规组(n=29)和试验组(n=29)。常规组实施锁定解剖钢板固定治疗,试验组患者实施逆行交锁髓内钉固定治疗,比较两组患者的症状治疗优良率、临床指标、恢复时间及治疗前、后自主生活能力评分。结果试验组的症状治疗优良率显著高于常规组(P<0.05)。试验组的住院时间、手术时间均显著短于常规组(P<0.05)。试验组的拆线时间、骨折愈合时间均显著短于常规组(P<0.05)。治疗后,两组的进食和平地行走评分均升高,且试验组高于常规组(P<0.05)。结论逆行交锁髓内钉应用于股骨下段及髁上骨折的治疗中可加快患者恢复速度,快速增强患者的自主生活能力,值得临床推广应用。Objective To analyze the clinical effect of retrograde interlocking intramedullary nails in the treatment of lower femoral and supracondylar fractures.Methods A total of 58 patients with lower femoral and supracondylar fractures who were treated in our hospital from January 2017 to October 2019 were selected as the research objects and divided into conventional group(n=29)and experimental group(n=29)according to the treatment method selected by the patients.The conventional group was treated with locking anatomical plate fixation,and the experimental group was treated with retrograde interlocking intramedullary nails.The excellent rate of symptomatic treatment,clinical indicators,recovery time and ability to live independently score before and after treatment were compared between the two groups.Results The excellent rate of symptomatic treatment in the experimental group was significantly higher than that in the conventional group(P<0.05).The hospitalization time and operation time in the experimental group were significantly shorter than those in the conventional group(P<0.05).The suture removal time and fracture healing time in the experimental group were significantly shorter than those in the conventional group(P<0.05).After treatment,the eating and walking scores in the two groups increased,and those in the experimental group were higher than the conventional group(P<0.05).Conclusion The retrograde interlocking intramedullary nails in the treatment of lower femoral and supracondylar fractures can speed up the recovery of patients and rapidly enhance the patients'ability to live independently,which is worthy of clinical promotion and application.
