185例中耳炎患者TMM检查结果分析

摘　　要：目的应用咽鼓管测压(Tubomanometry,TMM)检测185例中耳炎患者的咽鼓管评分(Eustachian tube Score,ETS),分析TMM在中耳炎诊治中的作用。方法以TMM检查法,检测185例不同类型中耳炎患者的咽鼓管(Eustachian tube,ET)功能,其中A组:慢性化脓性中耳炎静止期(以下简称为慢性化脓性中耳炎)80例(116侧),B组:分泌性中耳炎78例(128侧),C组:中耳胆脂瘤13例(16侧),D组:单纯鼓膜穿孔14例(15侧)。使用秩和检验比较每一组所有患耳与所有正常耳的ETS。结果A组患耳ETS为4.69±1.33,正常耳ETS为4.84±1.20,(PA=0.507);B组患耳ETS为3.64±1.89,正常耳ETS为4.61±1.50,(PB=0.013);C组患耳ETS为3.56±1.97,正常耳ETS为5.30±0.95,(PC=0.022);D组患耳ETS为4.53±1.19,正常耳ETS为4.85±1.14,(PD=0.489)。每一组正常耳的ETS均高于患耳。结论咽鼓管功能障碍(Eustachian tube dysfunction,ETD)与中耳炎的发生、发展关系密切;TMM是目前评价咽鼓管功能较为简单、经济、无创且准确的一种方法。Objective To report the eustachian tube score(ETS)in 185 patients with otitis media evaluated by Tubomanometry(TMM).Methods TMM was performed in 185 patients with various types of otitis media,including chronic suppurative otitis media(n=80,116 ears)(group A),secretory otitis media(n=78,128 ears)(group B),middle ear cholesteatoma(n=13,16 ears)(group C)and simple ear drum perforation(n=14,15 ears)(group D).ETS between diseased and normal ears was compared using the rank sum test.Result ETS was 4.69±1.33 for diseased ears and 4.84±1.20 for normal ears in group A(P=0.507);3.64±1.89 for diseased ears and 4.61±1.50 for normal ears in group B(P=0.013);3.56±1.97 for diseased ears and 5.30±0.95 for normal ears in group C(P=0.022);and 4.53±1.19 for diseased ears and 4.85±1.14 for normal ears in Group D(P=0.489).In all patients,ETS was higher in normal ears than in diseased ears.Conclusion Eustachian tube dysfunction is closely related to the occurrence and development of otitis media.TMM is a simple,economical,non-invasive and accurate method to evaluate eustachian tube function.
