小切口甲状腺手术治疗甲状腺良性肿瘤的临床效果

摘　　要：目的探讨小切口甲状腺手术治疗甲状腺良性肿瘤的临床效果。方法选取2016年12月~2018年12月萍乡市人民医院甲状腺外科进行诊治的122例甲状腺良性肿瘤行单侧腺叶切除术患者作为研究对象,根据手术切口的不同将患者分为对照组和观察组。对照组72例患者,实施传统甲状腺手术治疗,观察组50例患者,实施小切口甲状腺手术治疗。比较两组患者的术后疼痛评分;比较两组患者的手术指征(手术时间、术中出血量、术后引流时间、术后疼痛时间);比较两组患者的并发症(声音嘶哑、颈部压缩感、呼吸困难、切皮下结节、切口感染)总发生情况。结果观察组患者的术后疼痛评分低于对照组,差异有统计学意义(P<0.05)。两组患者的手术时间比较,差异无统计学意义(P>0.05)。观察组患者的术后引流时间、术后疼痛时间短于对照组,差异有统计学意义(P<0.05)。观察组患者的术中出血量少于对照组,差异有统计学意义(P<0.05)。观察组患者的并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论小切口甲状腺手术治疗甲状腺良性肿瘤的效果显著,具有创伤小、术后恢复快、美观、疼痛轻、安全性高等优点,值得临床推广使用。Objective To investigate clinical effect of small incision thyroidectomy in treatment for benign thyroid tumors.Methods A total of 90 patients with benign thyroid tumors who had undergone unilateral lobectomy treated in the Department of Thyroid Surgery of Pingxiang People's Hospital from December 2016 to December 2018 were selected as subjects.Patients were divided into control group and observation group according to the different surgical incision.In the control group,72 patients were treated with traditional thyroid surgery,and in the observation group,50 patients were treated with small incision thyroidectomy.Postoperative pain scores were compared between the two groups.Surgical indications(operative time,intraoperative bleeding volume,postoperative drainage time,postoperative pain time)were compared between the two groups.The total incidence of complications(hoarseness,neck compression,dyspnea,subcutaneous nodules,incision infection)were compared between the two groups.Results Postoperative pain score in observation group after operation was lower than that in control group,the difference was statistically significant(P<0.05).There was no difference in the operation time between the two groups(P>0.05).The postoperative drainage time and postoperative pain time in observation group were shorter than that in control group,the differences were statistically significant(P<0.05).The intraoperative bleeding volume in observation group was less than that in control group,the difference was statistically significant(P<0.05).The total incidence of complications in observation group was lower than that in control group,the difference was statistically significant(P<0.05).Conclusion Small incision thyroidectomy has significant effect in treatment for benign thyroid tumors,which has the advantages of small trauma,quick recovery,beautiful appearance,light pain,high safety.That is worthy of clinical application.
