针药结合治疗排卵障碍性不孕的临床效果

摘　　要：目的探讨针药结合治疗排卵障碍性不孕的促排卵的临床效果。方法选取2016年6月~2017年10月在广州中医药大学深圳医院就诊的96例排卵障碍性不孕患者,按照随机数字表法分为治疗组和对照组,每组各48例。对照组给予口服赖氏易孕方联合肌内注射人绒毛膜促性腺激素(HCG),治疗组在对照组治疗基础上联合通元针法干预。比较两组的临床效果、宫颈黏液评分、子宫内膜A型率、子宫内膜螺旋动脉搏动指数(PI)和阻力指数(RI)。结果两组的排卵率比较,差异无统计学意义(P>0.05);治疗组妊娠率为70.8%,明显高于对照组的43.8%,差异有统计学意义(P<0.01)。治疗组宫颈黏液评分和子宫内膜A型率均高于对照组,子宫内膜螺旋动脉搏动指数(PI)和阻力指数(RI)均低于对照组,差异有统计学意义(P<0.01)。结论针药结合可显著提高排卵障碍性不孕的促排卵效果,改善其宫颈黏液评分和子宫内膜容受性,调节生殖内分泌功能,提高妊娠率,效果优于单纯药物干预,值得临床推广应用。Objective To explore the clinical effect of acupuncture combined with Chinese herbal medicine in the treatment of ovulatory disorder infertility.Methods A total of 96 with ovulatory disorder infertility treated in Shenzhen Hospital of Guangzhou University of Chinese Medicine from June 2016 to October 2017 were enrolled.They were divided into treatment group and control group according to random number table method,48 cases in each group.In the control group,oral administration of Chinese herbal medicine Laishi Yiyun Formula combined with intramuscular injection of Human Chorionic Gonadotropin(HCG)were used.In the treatment group,on the basis of the control group,Tongyuan acupuncture was added.The clinical efficacy,cervical mucus score,endometrial type A rate,endometrial spiral artery pulsatility index(PI)and resistance index(RI)were compared between the two groups.Results There was no significant difference in ovulation rate between the two groups(P>0.05).The pregnancy rate in the treatment group was 70.8%,which was significantly higher than that of the control group accounting for 43.8%with statistical difference(P<0.01).The cervical mucus score and endometrial A rate in the treatment group were higher than those in the control group,and the endometrial spiral artery PI and RI were lower than those in the control group,and the differences were statistically significant(P<0.01).Conclusion The combination of acupuncture and Chinese herbal medicine can significantly enhance the ovulation-promoting effect on ovulatory disorder infertility,improve its cervical mucus score and endometrial receptivity,regulate its reproductive endocrine function,and increase its pregnancy rate.Its effect is better than Chinese herbal intervention alone,which is worthy of clinical promotion and application.
