摘 要:目的观察不同年龄段儿童反复呼吸道感染(recurrent respiratory tract infections,RRTIs)患儿血清维生素A、D的水平及其与疾病发生发展的关系。方法通过面对面调查问卷,从湖北省妇幼保健院门诊部门共选出1200例患儿。入组时间为2018年1月-2018年12月。受试者中,有600例患有RRTIs(RRTIs组),有600例患有RTIs(RTIs组)。两组受试者血液中维生素A含量均应用高效液相色谱法检测,25羟维生素D[25(OH)D]含量均采用化学发光微粒免疫法检测。结果RRTIs组和RTIs组中不同年龄段儿童的维生素A和25(OH)D水平具有显著性差异(P<0.05),其中0~2岁儿童的维生素A和25(OH)D水平最低。与RTIs组相同年龄段儿童相比,RRTIs组儿童的维生素A和25(OH)D水平均显著降低(P<0.05)。RTIs组维生素A、D不足或缺乏的发生率分别为34.3%、19.4%,而RRTI组发生率分别为63.0%、56.5%。两组维生素A、D不足或缺乏的发生率差异有显著性(P<0.05)。Logistic回归模型显示维生素A、D的不足或缺乏与RRTIs的发生显著正相关(P<0.05)。结论儿童呼吸道感染的发生发展与血清维生素A、D水平降低呈明显相关性,且儿童年龄越小,血清维生素A、D水平越低。Objective To investigate the levels of serum vitamin A and D in children with RRTIs and their relationship with the occurrence of the disease.Methods Through face-to-face questionnaires,a total of 1200 children were selected from the outpatient department of Hubei Maternal and Child Health Hospital.The enrollment time was from January 2018 to December 2018.Of the subjects,600 had RRTIs(RRTIs)and 600 had RTIs(RTIs).The content of vitamin A in blood of both groups was detected by high performance liquid chromatography.The content of 25-hydroxyvitamin D[25(OH)D]was detected by chemiluminescence microparticle immunoassay.Results There were significant differences in vitamin A and 25(OH)D levels between the RRTIs group and the RTIs group(P<0.05),among which vitamin A and 25(OH)D levels of children aged 0~2 were the lowest.Compared with children at the same age group in the RTIs group,the vitamin A and 25(OH)D levels in the RRTIs group were significantly lower(P<0.05).The incidence of insufficiency or deficiency for vitamins A and D was 34.3%and 19.4%in the RTIs group,but it increased to 63.0%and 56.5%in the RRTIs group,respectively.There were significant differences in the incidence of insufficiency or deficiency of vitamins A and D between the two groups(P<0.05).The logistic regression model showed that the insufficiency or deficiency of vitamins A and D was positively correlated with RRTIs occurrence(P<0.05).Conclusion The occurrence and development of respiratory infection in children is significantly correlated with the decrease of serum vitamin A and D levels,and the younger the children are,the lower the serum vitamin A and D levels are.
