能量多普勒超声在子宫黏膜下肌瘤和内膜息肉鉴别诊断中的应用

摘　　要：目的对比子宫内膜息肉和黏膜下肌瘤的能量多普勒超声声像图特征,探讨能量多普勒血流模式在子宫内膜病变鉴别诊断中的应用价值。方法选择2015年1-12月本院收治的98例子宫内膜息肉和黏膜下肌瘤患者作为研究对象,术前经阴道超声检查,采用B型超声和能量多普勒超声两种模式,根据能量多普勒超声结果进行诊断,与最终病理结果相对照。结果经术后病理证实子宫内膜息肉患者64例,黏膜下肌瘤患者34例。子宫内膜息肉患者中,54例(84.4%)表现为单血管模式,6例(9.4%)表现为多血管模式,2例(3.1%)表现为散在血管模式,2例(3.1%)未探及血流信号。子宫黏膜下肌瘤患者中,29例(85.3%)表现为环状血管模式,3例(8.8%)表现为多血管模式,2例(5.9%)表现为单血管模式。单血管模式诊断子宫内膜息肉的敏感性、特异性分别为84.4%和94.1%。环形血流模式诊断黏膜下肌瘤的敏感性、特异性分别为85.3%和100%。结论能量多普勒超声对子宫内膜病变有较高的诊断价值,可作为常规超声检查方法,尤其对子宫内膜息肉及黏膜下肌瘤的鉴别诊断具有明显优势,为临床诊断和治疗提供精确依据。Objective To compare characteristics of power Doppler mapping of endometrial polyps and submucosal myomas and analyze the application value of power Doppler flow in differential diagnosis of endometrial lesions.Methods 98 cases of patients suspected of having endometrial polyps or submucous myomas were enrolled in the study,all of them examined prospectively by transvaginal B-mode and power Doppler sonography.Suspected diagnoses of the lesions according to vascular Doppler characteristics were compared with the final diagnosis following histopathological examination.Results Patients included in the final analysis were 98 patients with histological confirmation of the type of endometrial lesion:64 with endometrial polyps and 34 with submucous myomas.Among the patients with endometrial polyps,54(84.4%)showed single vessel pattern,6(9.4%)showed multiple vessel pattern,2(3.1%)showed scattered vessel pattern,and 2(3.1%)without any blood flow signal.Of the patients with submucosal leiomyoma of the uterus,29(85.3%)showed annular vascular pattern,3(8.8%)showed multiple vascular pattern,and 2(5.9%)showed single vascular pattern.The sensitivity and specificity for single-vessel pattern in diagnosing endometrial polyps were 84.4%and 94.1%,and for annular pattern in diagnosing submucous myomas they were 85.3%and 100%,respectively.Conclusions Power Doppler ultrasound has high diagnostic value for endometrial lesions and could be used as a routine ultrasound examination method,especially for the differential diagnosis of endometrial polyps and submucosal leiomyoma,it could provide accurate basis for clinical diagnosis and treatment.
