摘 要:目的:探讨上、下肢主、被动踏车训练在早期偏瘫患者中的效果。方法:选择重庆三峡中心医院神经康复科收治的130例偏瘫患者作为研究对象,随机分为常规治疗组(65例,接受常规临床治疗及综合康复治疗)和踏车训练组(65例,在常规治疗组基础上增加上、下肢主、被动踏车训练),治疗4周后,对比分析两组患者治疗前后上、下肢运动功能、日常生活能力(ADL)及生活质量情况。结果:与治疗前比较,两组患者上、下肢运动功能评分及ADL评分均显著提高(P均=0.001);与常规治疗组比较,踏车训练组治疗后上、下肢运功功能评分[上肢:(32.44±7.81)分比(38.26±8.72)分,下肢:(18.33±5.18)分比(22.78±4.93)分]及ADL评分[(52.26±7.63)分比(59.72±8.84)分]提高更显著(P均=0.001);生活质量评分除了疼痛(BP)和功能(PF)两个维度外(P均>0.05),其他维度均表现为踏车训练组分值显著高于常规治疗组(P<0.05或<0.01)。结论:上、下肢主、被动踏车训练能显著提高早期偏瘫患者的运动功能和生活质量,更快地促进其生活自理。Objective:To explore application effect of upper and lower limb active and passive treadmill training in patients with early hemiplegia.Methods:A total of 130 hemiplegia patients in our department of neurological rehabilitation were randomly and equally divided into routine treatment group(received routine clinical treatment and comprehensive rehabilitative therapy)and treadmill training group(received upper and lower limb active and passive treadmill training based on routine treatment group).After four-week treatment,upper and lower limb motor function and ability of daily living(ADL)before and after treatment,and quality of life(QOL)were compared and analyzed between two groups.Results:Compared with before treatment,there were significant rise in scores of upper and lower limb motor function and ADL in two groups after treatment,P=0.001 all;compared with routine treatment after treatment,there were significant rise in scores of upper and lower limb motor function[upper limb:(32.44±7.81)scores vs.(38.26±8.72)scores,lower limb:(18.33±5.18)scores vs.(22.78±4.93)scores]and ADL[(52.26±7.63)scores vs.(59.72±8.84)scores]in treadmill training group,P=0.001 all;each dimension score of QOL in treadmill training group was significantly higher than that of routine treatment group except bodily pain(BP)and physical functioning(PF),P<0.05 or<0.01.Conclusion:Upper and lower limb active and passive treadmill training can significantly improve motor function and quality of life,and more quickly promote their self-care in patients with early hemiplegia.
