摘　　要：目的探讨早期营养支持对维持性血液透析患者氧化应激状态和生活质量的影响。方法选取2016年5月~2019年5月间我院收治的维持性血液透析患者101例,随机将患者分成研究组51例和对照组50例,对照组仅行常规治疗,研究组在此基础上开展早期营养支持,记录两组患者治疗前后氧化应激指标[晚期蛋白质氧化产物(AOPP)、血浆和红细胞丙二醛(MDA)、超氧化物歧化酶(SOD)]、不良事件(低血压、高血压、深静脉血流感染、心律失常、心绞痛、皮肤瘙痒等)发生情况,使用WHO生存质量测定量表(WHOQOL-BREF)评估患者的生活质量。结果与对照组相比,研究组患者氧化应激指标(AOPP、MDA)、不良事件发生情况明显降低,差异均具有统计学意义(P<0.05),SOD水平高于对照组(P<0.05)。与对照组相比,研究组患者总生活质量评分、社会关系领域、生理领域、心理领域、环境领域评分显著增高,差异具有统计学意义(P<0.05)。结论早期营养支持对维持性血液透析患者具有重要影响,不仅降低了患者透析期间的氧化应激反应,缓解患者的心理生理负担,同时通过营养的干预,提高患者自身免疫功能,提升患者的生活质量,值得在临床上推广。Objective To investigate early nutritional support on oxidative stress and quality of life in patients on maintenance hemodialysis.Methods A total of 101 patients on maintenance hemodialysis who were admitted inour hospital from May 2016 to May 2019 were selected.They were randomly divided into the research group(n=51)and the control group(n=50).Patients in the control group merely underwent the routine treatment,whilethose in the research group additionally accepted early nutritional support.Baseline and post-treatment indices of oxidative stress incorporating advanced oxidation protein products(AOPP),malondialdehyde(MDA)and superoxide dismutase(SOD),and incidence rates of adverse events encompassing hypotension,hypertension,deep venous catheter-related bloodstream infection,arrhythmia,angina pectoris and pruritus in the two groups were recorded.The World Health Organization Quality of Life study assessment instrument(WHOQOL-Bref)was used to assess quality of life in all patients.Results Compared with the control group,two indices of oxidative stress,AOPP and MDA levels,and the incidence rate of adverse events were significantly reduced in the research group(P<0.05).Its SOD level was significantly higher than that in the control group(P<0.05).The total score of WHOQOL-BREF and the score of each part(social relations,physiology,psychology and environment)in the research group were higher than those in the control group,with statistically significant differences(P<0.05).Conclusion Early nutritional support is of critically significance for patients on maintenance hemodialysis.Despite an amelioration of oxidative stress response,it can relieve psychological and physiological burdens patients undertaken and promote autoimmune response with improved quality of life.Therefore,the application of early nutritional support is worth promoting.
