IGF-Ⅰ、IGFBP-3血清指标联合检测在脑肿瘤的诊断应用价值

摘　　要：目的 探讨研究IGF-Ⅰ、IGFBP-3指标水平对各种类型脑肿瘤的诊断价值.方法 选取2015年3月至2018年6月收治的脑肿瘤患者共77例,其中脑垂体瘤52例,根据分型不同分为两组,其中A组22例,为GH分泌型、B组30例为非GH分泌型;另外脑膜瘤14例,设为C组,胶质瘤11例,设为D组;另选取同期体检中心健康者共20例作为对照,设为E组.于清晨在所有患者空腹时抽取静脉血约4ml,应用分析仪进行检测.结果 A组、B组、C组患者的IGF-Ⅰ水平均明显高于E组,其中A组患者的IGF-Ⅰ水平最高(P<0.05);A组、B组、C组、D组的IGFBP-3水平均明显高于E组,其中A组患者的IGFBP-3水平最高(P<0.05);A组患者手术治疗后7天的IGF-Ⅰ、IGFBP-3指标水平明显低于手术治疗前(P<0.05),而B组、D组患者手术治疗后的7天的IGF-Ⅰ、IGFBP-3指标水平与手术治疗前无明显差异(P>0.05).结论 IGF-Ⅰ、IGFBP-3均对脑肿瘤具有辅助诊断作用,尤其是前者,在辨别不同类型脑肿瘤方面作用显著;另外对于GH分泌型脑垂体瘤患者,术后检测IGF-Ⅰ可以有效评估手术的治疗效果,且对其术后恢复评估也具有一定的作用.Objective To explore the diagnostic value of IGF-Ⅰand IGFBP-3 index levels for various types of brain tumors.Methods A total of 77 patients with brain tumors admitted from March 2015 to June 2018 were selected,including 52 cases of pituitary tumor,14 cases of meningioma,which were set as group C,11 cases ofglioma,which were set as group D.The 52 cases of pituitary tumor were divided into two groups according to different types,including 22 cases in group A,which were GH secreting type,and 30 cases in group B were non-GH secreting type.Another 20 cases of healthy people in the physical examination center during the same period were selected as control group,which wereset as group E.In the early morning,about 4 ml of venous blood was drawn from all patients on an empty stomach and were tested for analyze.Results The levels of IGF-I inpatients in groups A,B,and C were significantly higher than those in group E,and with the highest level of IGF-Ⅰin patients in group A(P<0.05).The IGFBP-3 levels of A,B,C and D groups were significantly higher than those of the group E,and in group A patients was the highest(P<0.05).The levels of IGF-Ⅰand IGFBP-3 in group A was significantly decreased 7 days after surgery(P<0.05),but no changes in groups B and D(P>0.05).Conclusion IGF-Ⅰand IGFBP-3 have an auxiliary diagnostic effect on brain tumors,especially the IGF-Ⅰ,which has a significant role in distinguishing different types of brain tumors.In addition,for patients with GH secretory pituitary tumors,postoperative detection of IGF-Ⅰcan be effective evaluate the therapeutic effect of surgery,and also have a certain role in the evaluation of postoperative recovery.
