摘　　要：目的探讨临床药师参与院内静脉血栓栓塞症(Venous thromboembolism,VTE)防控体系构建的工作模式及作用。方法临床药师利用药学专业知识,从多方面开展院内VTE防控体系构建中的药学实践工作。结果临床药师可以通过参加多学科会诊、点评重点住院病例医嘱、药学查房、合理利用基因检测、参与临床培训巡讲等工作,参与VTE防控体系构建中的药学实践,协助医生调整用药方案、给予合理用药建议,并获得了临床科室认可。结论初步形成了临床药师参与院内VTE防控构建的工作模式。Objective To explore the working mode and role of clinical pharmacists in the prevention and control system of venous thromboembolism(VTE)in hospitals.Methods Clinical pharmacists participated in the pharmaceutical practice of prevention and control system VTE from many aspects by using professional knowledge.Results Clinical pharmacists participated in multidisciplinary consultation,review of doctor’s advice for key inpatients,pharmaceutical ward round,reasonable use of genetic testing and the clinical training tour,assisted doctors to adjust the medical treatment scheme,give advice for the rational use of drugs,and got the approval of clinical departments.Conclusion The working mode of clinical pharmacists participating in the prevention and control system of VTE in hospital was preliminarily formed.
