摘  要:目的比较显影线超声定位技术与常规超声引导技术在实习医生桡动脉穿刺中的应用效果。方法选择72例在全身麻醉下进行肝脏手术或脾脏切除且拟行桡动脉穿刺的患者。使用密封的信封方法,将患者随机分为常规超声(A)组和显影线超声定位(B)组。比较2组患者穿刺情况(包括超声定位时间、首次穿刺成功率和穿刺失败率)和2组血管并发症(包括出血、血肿、血管比赛和动脉瘤)发生情况。结果B组桡动脉首次穿刺成功率明显高于A组(74.3%比30.5%),超声定位时间明显短于A组[(6.8±3.2)s比(17.4±8.5)s](均P<0.01);2组穿刺失败率比较差异无统计学意义(P>0.05)。B组穿刺部位出血发生率明显低于A组(17.1%比47.2%,P<0.01)。结论显影线超声定位技术用于实习医生桡动脉穿刺能显著提高首次穿刺成功率和缩短超声定位时间。Objective To compare the efficacies of development line ultrasound positioning technology and conventional ultrasound guidance technique in radial artery puncture in interns.Methods Seventy-two patients who underwent liver surgery or spleen resection under general anesthesia and were scheduled for radial artery puncture were selected in this study.These patients were randomized into two groups using the sealed envelope method:conventional ultrasound group(group A)and development line ultrasound positioning group(group B).Puncture situation(positioning time,first-attempt success rate and failure rate)and complications(hemorrhage,hematoma,arterial occlusion and aneurysm were compared between the two groups.ResultsCompared with conventional ultrasound guidance,development line ultrasound positioning increased the first-attempt puncture success rate(74.3%vs.30.5%),shortened the positioning time((6.8±3.2)s vs.(17.4±8.5)s,and reduced the incidence of hemorrhage(17.1%vs.47.2%)(P<0.01).There was no significant difference in the failure rate of puncture between the two groups(P>0.05).Conclusion The development line ultrasound positioning technology can improve the first-attempt success rate of radial artery puncture and shorten the ultrasound positioning time in interns.
