95例经典型不明原因发热的病因及临床特点分析

摘　　要：目的分析95例经典型不明原因发热的病因及临床特点。方法对该院2016年1月—2018年12月接收的95例经典型不明原因发热患者临床资料回顾性分析。结果95例患者中,感染性疾病、结缔组织、恶性肿瘤、杂病类及病因未明分别有56例、16例、7例、9例和7例,占比分别是58.95%、16.84%、7.37%、9.47%和7.37%;感染性疾病主要是男性,大部分患者存在头痛及畏寒。结缔组织病中,男性与女性患病概率差异无统计学意义(P>0.05),该类患者大部分存在淋巴结肿大、皮疹、肌肉痛及关节痛;相比于恶性肿瘤及感染性疾病,结缔组织病中性粒细胞百分比、白细胞计数、SF及ESR提高明显,差异有统计学意义(P<0.05);恶性肿瘤患病人群以超过40岁人为主,大多数是男性患者,脾大且纳差明显,差异有统计学意义(χ^2=2.514,P=0.002)。结论感染性疾病是不明原因发热主要因素,其次包括恶性肿瘤、杂病类及结缔组织病。对于具体病种来讲,经典型不明原因发热常见疾病是AOSD,其次包括淋巴瘤和伤寒。根据具体病症、实验室检查结果等开展临床诊治,有助于治疗效果的提高,确保良好的治疗效果,使患者及早恢复健康。Objective To analyze the etiology and clinical features of 95 cases of unexplained fever.Methods A retrospective analysis of 95 clinical data of classic unexplained fever patients received from January 2016 to December 2018 in the hospital.Results Among the 95 patients,56 cases,16 cases,7 cases,9 cases and 7 cases were found to be infectious diseases,connective tissues,malignant tumors,miscellaneous diseases and etiology,accounting for 58.95%,16.84%and 7.37%,9.47%,and 7.37%;infectious diseases are mainly male,and most patients have headaches and chills.In connective tissue diseases,there is no statistically significant difference in the probability of male and female disease(P>0.05).Most of these patients have lymphadenopathy,rash,muscle pain and joint pain.Compared with malignant tumors and infectious diseases,connective tissue disease neutral granules percentage of cells,white blood cell count,SF and ESR increased significantly.After statistical analysis,the difference was statistically significant.The patients with malignant tumors were mainly over 40 years old,most of them were male patients,and the spleen was large and the anorexia was obvious,and the difference was statistically significant(χ^2=2.514,P=0.002).Conclusion Infectious diseases are the main cause of unexplained fever,followed by malignant tumors,miscellaneous diseases and connective tissue diseases.For specific diseases,the common type of common disease with unexplained fever is AOSD,followed by lymphoma and typhoid.According to the specific conditions,laboratory results,etc.,clinical diagnosis and treatment can help improve the treatment effect,ensure a good therapeutic effect,and restore the patient's health early.
