摘 要:目的探讨体外振动排痰机在危重症患者排痰方法中的临床效果。方法方便选择在该院重症医学科2017年6月—2019年6月收治的肺部感染且因病情关系需行机械通气患者共100例,给予抗感染、药物祛痰、营养支持等常规治疗。将患者分两组:对照组50例采取人工叩背排痰方法进行排痰治疗,观察组50例采用体外振动排痰机进行排痰治疗。观察并比较两组患者的呼吸功能指标(Raw、PaO2/FiO2和PaCO2)、排痰量、吸痰次数、排痰效果、机械通气时间、ICU住院时间以及并发症发生情况。结果治疗后两组患者的Raw和PaCO2均较治疗前显著下降(P<0.05),PaO2/FiO2较治疗前显著上升(P<0.05),且观察组改善程度显著优于对照组(P<0.05);观察组的排痰量显著多于对照组(P<0.05),且吸痰次数显著少于对照组(P<0.05);观察组患者排痰有效率显著高于对照组(94.00%vs 76.00%)(χ^2=6.352,P<0.05);观察组的机械通气时间和ICU住院时间均显著短于对照组(P<0.05);观察组患者并发症发生率显著低于对照组(P<0.05)。结论使用体外振动排痰机给危重症患者排痰,可显著改善呼吸功能,有效提高排痰效果,减少吸痰次数,降低炎症指标和并发症发生率,具有一定的临床实用价值。Objective To investigate the clinical effect of external vibratory sputum excretion machine in the method of sputum excretion in critically ill patients.Methods Convenient elect a total of 100 patients with pulmonary infection who were admitted to the Department of Critical Care Medicine from June 2017 to June 2019 in this hospital were enrolled in this study.The patients were treated with anti-infective,drug-induced sputum and nutritional support.The patients were divided into two groups:50 patients in the control group were treated with artificial sputum drainage and sputum drainage.50 patients in the observation group were treated with external sputum drainage machine for sputum drainage.Observe and compare respiratory function parameters(Raw,PaO2/FiO2,and PaCO2),sputum volume,frequency of sucking,sputum drainage,mechanical ventilation time,ICU hospitalization time,and complications.Results After treatment,Raw and PaCO2 were significantly lower than those before treatment(P<0.05),PaO2/FiO2 was significantly higher than before treatment(P<0.05),and the improvement of observation group was significantly better than that of control group(P<0.05).The amount of sputum in the observation group was significantly higher than that in the control group(P<0.05),and the number of sucking was significantly less than that in the control group(P<0.05).The effective rate of sputum sputum in the observation group was significantly higher than that in the control group(94.00%vs 76.00%)(χ^2=6.352,P<0.05);mechanical ventilation time and ICU hospitalization time in the observation group were significantly shorter than the control group(P<0.05);the incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusion The use of external vibrating sputum sputum in patients with critically ill patients can significantly improve respiratory function,effectively improve the effect of sputum excretion,reduce the number of sputum,reduce the incidence of inflammatory indicators and
