摘 要:目的研究Notch信号转导通路与IgA肾病患者各临床指标之间的意义。方法实验组方便选取包头医学院第一附属医院肾脏内科2013年1月-2014年10月确诊的IgA肾病的患者36例,根据IgAN牛津病理分型肾小管萎缩或间质纤维化(T)结果将实验组分为N1、N2、N3组。对照组方便选取包头医学院第一附属医院泌尿外科肾肿瘤患者。检测所有患者的各种生化指标,运用酶联免疫吸附法检测IgAN患者血中的Notch1、Jagged1水平。结果 24 h尿蛋白定量的结果在N1(2.05±1.75)g/d、N2(2.27±0.99)g/d、N3(2.46±1.58)g/d组与C组(0.30±0.10)g/d间比较,均差异有统计学意义(P<0.05)。ALB、CHO、TG、HDL-C、LDL-C、UA的结果在N1、N2、N3组与C组间比较,差异无统计学意义(P>0.05),Scr的结果在N1与C组间比较,差异无统计学意义(P>0.05);N2、N3组与C组间比较,差异有统计学意义(P<0.05)。血Notch1水平在对照组C和IgA肾病各组N1、N2、N3分别为(0.43±0.24)、(1.19±0.38)、(1.54±0.46)、(3.77±2.99)。血Jagged1水平在对照组C和IgA肾病各组N1、N2、N3分别为(1.46±0.32)、(2.36±0.43)、(2.44±0.88)、(5.06±2.96)。血Notch1、Jagged1水平在实验组与对照组C比较均增高,差异有统计学意义(P<0.05)。结论提示肾脏的Notch1/Jagged1信号转导通路在IgA肾病时可能被激活,参与疾病的进一步发生和发展。Objective The link between IgA nephropathy pathogenesis and Notch signaling pathway. Methods The experimental group was convenient 36 patients with IgA nephropathy from the Department of Nephrology from January 2013 to October 2014, the First Affiliated Hospital of Baotou Medical College. The experimental components were grouped into N1,N2, and N3 groups based on the results of IgAN Oxford pathological classification of renal tubular atrophy or interstitial fibrosis(T). The control group was the normal tissues adjacent to the tumor in patients with renal tumors in the urology department of the First Affiliated Hospital of Baotou Medical College. Various biochemical indicators of all patients were detected, and the levels of Notch1 and Jagged1 in the blood of IgAN patients were detected by enzyme-linked immunosorbent assay. Results The results of 24 h urine protein quantification were statistically significant between the N1(2.05±1.75)g/d,N2(2.27±0.99)g/d, N3(2.46±1.58)g/d and C groups(0.30±0.10)(P<0.05). The results of Hb, ALB, CHO, TG, HDL-C,LDL-C, and UA were not statistically significant between the N1, N2, and N3 groups and the C group(P>0.05). The results of Scr were compared between the N1 and C groups. The difference was not statistically significant(P>0.05);there was statistical significance between the N2 and N3 groups and the C group(P<0.05). The levels of blood Notch1 in the control group C and IgA nephropathy groups N1, N2, N3 were(0.43±0.24),(1.19±0.38),(1.54±0.46),(3.77±2.99). The levels of Jagged1 in blood were(1.46 ±0.32),(2.36 ±0.43),(2.44 ±0.88),(5.06 ±2.96) in the control group C and IgA nephropathy in each group. The levels of blood Notch1 and Jagged1 were increased in the experimental group and the control group C, and the difference was statistically significant(P <0.05). Conclusion In patients with IgA nephropathy, clew kidney Notch1/Jagged1 signal transduction pathway may be activated when the IgA nephropathy, participate in the further occurrence and development of the disease
