外固定支架固定与切开复位内固定对桡骨远端骨折患者腕关节功能的影响

摘　　要：目的对比外固定支架固定与切开复位内固定对桡骨远端骨折患者腕关节功能的影响。方法选取2016年6月至2018年5月本院收治的桡骨远端骨折患者86例,利用随机数表法分为两组,各43例。对照组行切开复位内固定,观察组行外固定支架固定术,对比两组尺偏角、掌倾角、腕关节功能及并发症发生情况。结果术前、术后3个月及6个月,两组尺偏角及掌倾角对比差异无统计学意义;术后3、6个月,两组尺偏角较术前高,掌倾角较术前低,差异有统计学意义(P<0.05);术后3个月,观察组Mayo评分高于对照组,差异有统计学意义(P<0.05);术后6个月,两组Mayo评分比较差异无统计学意义;观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论外固定支架固定与切开复位内固定治疗桡骨远端骨折均可取得良好成效,但外固定支架固定术对患者近期腕关节功能的影响显著,且可有效降低并发症发生率。Objective To compare the effect of external fixator fixation and open reduction and internal fixation on wrist joint function in patients with distal radius fracture.Methods 86 patients with distal radius fracture who admitted in the hospital from June 2016 to May 2018 were selected,and they were divided into two groups according to the random number table method,with 43 cases in each group.Control group underwent open reduction and internal fixation,and observation group was treated with external fixator fixation.The ulnar inclination,volar tilting angle,wrist joint function,and the incidence of complications were compared between the two groups.Results Before surgery,after surgery for 3 months and 6 months,there was no statistical difference in the ulnar inclination and volar tilting angle between the two groups.After surgery for 3 months and 6 months,the ulnar inclination of two groups was higher than that before surgery,and the volar tilting angle was lower than that before surgery(P<0.05).After surgery for 3 months,the Mayo score of observation group was higher than that of control group(P<0.05).After surgery for 6 months,there was no statistical difference in the Mayo score between the two groups.The incidence of complications in observation group was lower than that in control group(P<0.05).Conclusion External fixator fixation and open reduction and internal fixation in treatment of distal radius fracture can achieve good efficacy,but external fixator fixation has significant effect on short-term wrist joint function of patients,and it can effectively decrease the incidence of complications.
