摘 要:目的分析晚断脐法对早产儿血红蛋白及胆红素的影响,以期改善早产儿发病境况。方法选择2017年10月至2019年3月于本院产科出生的60例胎龄32~35周的早产儿作为研究对象,随机分为对照组和观察组,每组30例。对照组采用传统断脐法,断脐时间为早产儿出生后10 s;观察组采用晚断脐法,断脐时间:早产儿出生后3 min。比较两组早产儿在出生24 h后血红蛋白水平及出生后出现胆红素峰值的时间。结果观察组早产儿血红蛋白水平在出生时和出生24 h后分别为(163.87±13.08)g/L,(189.77±13.16)g/L,显著高于对照组的(156.97±17.26)g/L、(160.43±16.89)g/L;两组均在出生后5 d左右出现胆红素峰值,比较差异无统计学意义。结论对早产儿实施晚断脐能够显著增加血红蛋白水平,但对胆红素的产生并无明显影响。早产儿采用晚断脐法,可得到更多血供应及铁积存量,增强了早产儿心肺系统顺应性,提高了机体的免疫功能,减少各种合并症发生,改善早产儿境况,具有较大的临床应用空间。Objective To analysis the effect of late umbilical cord transection on hemoglobin and bilirubin in premature infants in order to improve the incidence of premature infants.Methods From October 2017 to March 2019,60 premature infants with gestational age of 32 weeks to 35 weeks were selected and they were randomly divided into two groups,with 30 cases in each group.The control group was treated with traditional umbilical cord breaking method,the umbilical cord breaking time was 10 s after birth,and the observation group was treated with late umbilical cord cutting method,and the umbilical cord breaking time was 3 min after birth.The hemoglobin level and the peak time of bilirubin were compared between the two groups 24 h after birth.Results The hemoglobin levels in observation group of premature infants at birth and 24 h after birth were(163.87±13.08)g/L and(189.77±13.16)g/L,respectively,which were significantly higher than those in the control group(156.97±17.26)g/L and(160.43±16.89)g/L,respectively,and there was no significant difference between the two groups at about 5 days after birth.Conclusion Late umbilical cord transection can significantly increase hemoglobin level in premature infants,but has no significant effect on the production of bilirubin.Premature infants were cut off by late umbilical cord method,but more blood supply and iron accumulation have been obtained,which can enhance the compliance of cardiopulmonary system of premature infants,improve the immune function of the body,reduce the occurrence of various complications,improve the situation of premature infants,and have great clinical application space.
