孕早期彩色多普勒超声联合孕中期母体血清标志物检测在产前筛查胎儿先天性心脏病中的价值观察

摘　　要：目的探讨孕早期彩色多普勒超声联合孕中期母体甲胎蛋白(AFP)、游离β绒毛膜促性腺激素(free-β-HCG)检测在产前筛查胎儿先天性心脏病中的价值。方法选取2017年1月至2018年1月本院收治的65例具有可疑征象胎儿为研究对象,所有孕妇均行彩超检查及母体血清检测,以足月产或引产结果为金标准,比较彩超、母体血清检测及两者联合诊断胎儿先天性心脏病的临床价值。结果彩超检查的灵敏度及特异度均显著高于母体血清检测(P<0.05);而两者联合诊断的AUC为0.869,显著高于超声单独检查及母体血清单独检测(P<0.05)。结论孕早期彩色多普勒超声联合孕中期母体血清检测能够有效提高产前筛查胎儿先天性心脏病的准确率,具有一定临床诊断价值。Objective To investigate the value of early pregnancy color doppler ultrasound combined with metaphase pregnancy detection of maternal alpha-fetoprotein(AFP)and freeβ-human chorionic gonadotropin(free-β-HCG)in the prenatal screening of fetal congenital heart disease.Methods 65 fetuses with suspicious signs who were treated in our hospital from January 2017 to January 2018 were selected for the study.All pregnant women were given color doppler ultrasound and maternal serum detection.The results of full-term or induction labor were taken as gold standard to compare the clinical value of color doppler ultrasound,maternal serum detection and the combination of the two in the diagnosis of fetal congenital heart disease.Results The sensitivity and specificity of color doppler ultrasound examination were significantly higher than those of maternal serum detection(P<0.05).The AUC of the combination of the two was 0.869,which was significantly higher than that of ultrasound alone and maternal serum detection alone(P<0.05).Conclusion Early pregnancy color doppler ultrasound combined with metaphase pregnancy maternal serum detection can effectively improve the accuracy rate of prenatal screening for fetal congenital heart disease,and has certain clinical diagnostic value.
