LISS钢板与ORIF术治疗胫骨平台骨折的效果比较及对开始负重时间和软骨细胞凋亡的影响研究

摘　　要：目的探讨LISS钢板与ORIF术治疗胫骨平台骨折的效果比较及对开始负重时间、软骨细胞凋亡的影响。方法选取2015年5月至2018年5月于本院进行手术治疗的胫骨平台骨折患者120例,采用随机分组方式分为研究组和对照组,每组60例。对照组进行ORIF治疗,研究组进行LISS内固定治疗,研究对比两组患者各手术指标、并发症以及膝关节评分的差异。结果研究组术中出血量、手术时间以及骨折愈合时间等指标显著优于对照组,负重训练开始时间早于对照组(P<0.05);术前两组患者软骨细胞凋亡情况比较差异无统计学意义,术后3个月研究组凋亡情况显著优于对照组(P<0.05);随访3个月后研究组膝关节以及放射学评分均显著优于对照组(P<0.05),研究组和对照组治疗后并发症的发生率分别为1.7%和11.7%,研究组发生率显著低于对照组(P<0.05)。结论LISS技术治疗胫骨平台骨折效果显著,可提升手术效率,减少术中出血量,减少软组织细胞的凋亡,有助于患者术后的恢复。Objective To investigate the comparison of the effects of LISS plate and ORIF in the treatment of tibial plateau fractures and the effect on the time of weight bearing and chondrocyte apoptosis.Methods A total of 120 patients with tibial plateau fractures treated in our hospital from May 2015 to May 2018 were randomly divided into study group and control group,with 60 cases in each group.The control group was treated with ORIF,and the study group was treated with LISS internal fixation.The differences in surgical indicators,complications,and knee joint scores were compared between the two groups.Results The indexes such as blood loss,operation time and fracture healing time in the study group were significantly better than those in the control group,and the weight training started earlier than the control group(P<0.05).There was no significant difference in the chondrocyte apoptosis between the two groups of patients before the operation,the apoptosis of the study group was significantly better than that of the control group at 3 months after operation(P<0.05).The knee and radiological scores of the study group were significantly better than those of the control group at 3 months of follow-up(P<0.05).The incidence of complications after treatment in the control group was 1.7%and 11.7%,respectively,and the incidence in the study group was significantly lower than that in the control group(P<0.05).Conclusion LISS technology has a significant effect on tibial plateau fractures,which can improve the efficiency of surgery,reduce the amount of bleeding during the operation,reduce the apoptosis of soft tissue cells,and help patients recover after surgery.
