脓毒症CRRT治疗患者血降钙素原与APACHE评分的关系

摘　　要：目的探讨CRRT治疗的脓毒症患者APACHE评分和生化指标的关系。方法选取2015年11月至2017年3月本院行CRRT治疗的脓毒症患者55例,根据APACHE评分分为差、中、优组,比较3组间血生化指标血肌酐、尿素氮、乳酸、血钾和降钙素原(PCT)的水平,Spearman相关分析APACHE评分与上述指标的相关性。结果APACHE评分优组血肌酐、尿素氮和乳酸水平均较低,与差、中组比较差异有统计学意义(P<0.05);APACHE评分差、中、优组血钾水平和降钙素原水平均逐渐降低,组间比较差异有统计学意义(P<0.05);Spearman等级相关分析显示,APACHE评分得分与血钾和降钙素原存在密切相关性(P<0.01),其余指标间未见显著相关性。结论脓毒症CRRT治疗的患者APACHE评分可以在一定程度上反映患者体内生化指标的改善情况,与体内免疫稳态存在对应关系。Objective To study the relationship between APACHE score and biochemical index for patients treated by the CRRT.Methods 55 sepsis patients from November 2015 to March 2017 in CRRT treatment were randomly divided into poor,medium and good group according to the APACHE score difference.The level of blood creatinine,urea nitrogen,lactic acid,blood potassium and calcitonin(PCT)of the patients in three group was compared.Spearman correlation analysis was used to analyze the relationship between the APACHE score and these indicators.Results The level of blood creatinine,urea nitrogen,lactic acid of the patients in good group were lower,it was statistically significant difference compared with other two group(P<0.05).The level of blood potassium and PCT of the patients in three groups were gradually reduced,the difference compared with each other was statistically significant(P<0.05).Spearman rank correlation analysis showed that the APACHE score was close correlation with potassium and PCT(P<0.01),and there was no significant correlation between each index.Conclusion The APACHE score of sepsis patients in CRRT treatment can reflect the situation of their biochemical indexes in somehow,and it is in corresponding relation with immune homeostasis.
