摘　　要：目的探讨甲氨蝶呤(MTX)妊娠囊内注射联合清宫术治疗剖宫产术后子宫瘢痕妊娠的可行性。方法选择2014年3月至2017年9月本院妇科接诊的80例剖宫产术后子宫瘢痕妊娠患者,随机分为治疗组和对照组各40例。对照组患者采用MTX囊内注射治疗,治疗组患者在对照组基础上联合清宫术治疗,观察两组患者临床相关时间指标及预后,评估临床疗效。结果治疗组治疗总有效率高于对照组(P<0.05);对照组血β-HCG恢复时间、病灶消失时间长于治疗组(P<0.05),两组患者月经恢复正常时间比较差异无统计学意义(P>0.05);对照组并发症总发生率高于治疗组(P<0.05)。结论甲氨蝶呤妊娠囊内注射联合清宫术治疗剖宫产术后子宫瘢痕妊娠效果显著,具有住院时间短、促进血β-HCG水平降低、帮助月经恢复、安全性高等优点,值得临床推广。Objective To investigate the clinical efficacy of methotrexate(MTX)gestational injection combined with complete curettage of uterine cavity in the treatment of caesarean scar pregnancy,and to analyze the effect on the clinical index and prognosis.Methods A total of 80 patients with caesarean scar pregnancy enrolled in our hospital from March 2014 to September 2017 were selected and randomly divided into observation group and control group,with 40 cases in each group.The patients in the control group were treated with MTX intracapsular injection,while the patients in the observation group were treated with the complete curettage of uterine cavity on the basis of the control group.The clinical related time indicators and prognosis of the two groups were observed,and the clinical efficacy was evaluated.Results The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).The recovery time of the bloodβ-HCG and the disappearance time of the lesion in the control group were significantly longer than those in the observation group(P<0.05).There was no significant difference in the recovery time of menstruation between the two groups(P>0.05).The overall incidence of complications in the control group was significantly higher than that in the observation group(P<0.05).Conclusion The effect of methotrexate gestational injection combined with complete curettage of uterine cavity in the treatment of caesarean scar pregnancy is significant,with the advantages of short hospitalization time,reducing the blood level ofβ-HCG,helping promote menstruation recovery and high security,which has high clinical value.
