基于度量衡研究清胃散剂量刍议

摘　　要：目的:探索清胃散临床应用剂量。方法:通过查阅度量衡文献以推断出清胃散的理论剂量;通过收集临床使用清胃散个案,运用现代统计方法,以得出清胃散现代应用之置信区间,并将两者结果进行比较。结果:根据度量衡推断出的理论值均按照升麻为1的系数进行比例换算,其余药物的置信区间为生地黄1.7103~2.2860,当归1.1160~1.4644,牡丹皮1.2105~1.4704,黄连0.7507~0.9774。通过统计清胃散临床个案使用剂量为:生地黄14.86~19.33 g,当归9.53~11.47 g,牡丹皮10.3~12.46 g,黄连6.5~8.45 g,升麻8.07~10.59 g。结论:通过度量衡推断出的清胃散剂量比例与通过临床个案统计得出的清胃散剂量比例的置信区间相差甚远,但临床运用时则不必悉具,而应辨证论治,随证加减,灵活运用。Objective:To observe the clinical dosage of Stomach-Heat-Clearing Powder.Methods:The theoretical dosage of Stomach-Heat-Clearing Powder was deduced through consulting the literature of weights and measures.The confidence interval of modern application of Stomach-Heat-Clearing Powder was obtained through collecting clinical cases,and then the two results were compared.Results:The theoretical values deduced from the weights and measures were all scaled according to the coefficient of 1 for Shengma(Rhizoma Cimicifugae).The confidence intervals of other medicinals were as follows:1.7103-2.2860 for Shengdi(Radix Rehmanniae),1.1160-1.4644 for Danggui(Radix Angelicae Sinensis),1.2105-1.4704 for Mudanpi(Cortex Moutan),0.7507-0.9774 for Huanglian(Rhizoma Coptidis).The confidence intervals of clinical dosage through collecting clinical cases were as follows:14.86-19.33 g for Shengdi(Radix Rehmanniae),9.53-11.47 for Danggui(Radix Angelicae Sinensis),10.3-12.46 for Mudanpi(Cortex Moutan),6.5-8.45 g for Huanglian(Rhizoma Coptidis),and 8.07-10.59 g for Shengma(Rhizoma Cimicifugae).Conclusion:The confidence interval of Stomach-Heat-Clearing Powder dosage proportion deduced from weights and measures is far from that obtained from clinical case statistics.However,in clinical application,it is not necessary to fully follow this,but we should flexibly use it according to syndrome differentiation,with proper addition and subtraction.
