益精补阳还五汤联合曲伏前列素滴眼液治疗原发性开角型青光眼35例

摘　　要：目的:观察益精补阳还五汤联合曲伏前列素滴眼液治疗原发性开角型青光眼的临床疗效及其对患者视功能的影响。方法:将70例原发性开角型青光眼患者按照随机数字表法分为对照组和观察组,每组各35例。对照组给予曲伏前列素滴眼液治疗,观察组在对照组的基础上加用益精补阳还五汤。比较两组的临床疗效及治疗前后中医证候积分变化情况,检测两组患者治疗前后视功能指标(视力、眼压、视野缺损范围、视野光敏度)、视网膜中央动脉血流[收缩期血流峰值速度(systolic peak velocity,SPV)、舒张末期血流速度(end diastolic velocity,EDV)、阻力指数(resistance index,RI)]变化情况。结果:观察组有效率为88.57%,对照组有效率为68.57%,两组患者有效率比较,差异具有统计学意义(P<0.05)。两组患者中医证候积分低于同组治疗前,且观察组治疗后低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后视力、视野光敏度高于同组治疗前,眼压、视野缺损范围低于同组治疗前,观察组患者治疗后视力、视野光敏度高于对照组治疗后,眼压、视野缺损范围低于对照组治疗后,差异具有统计学意义(P<0.05)。两组患者治疗后SPV、EDV高于同组治疗前,RI低于同组治疗前,差异具有统计学意义(P<0.05);观察组患者治疗后SPV、EDV高于对照组治疗后,RI低于对照组治疗后,差异具有统计学意义(P<0.05)。观察组不良反应发生率为25.71%,对照组不良反应发生率为54.29%,两组患者不良反应发生率比较,差异具有统计学意义(P<0.05)。结论:益精补阳还五汤联合曲伏前列素滴眼液可改善原发性开角型青光眼患者的临床症状,同时可降低眼压、改善视网膜中央动脉血流参数。Objective:To observe the clinical curative effect of Yijing Buyang Huanwu Decoction(益精补阳还五汤,YJBYHWD)in combination with Travoprost Eye Drops on open angle glaucoma and its influence on visual function of patients.Methods:A total of 70 patients with open-angle glaucoma were divided into the control group and the observation group according to the random number table method,with 35 cases in each group.The control group was treated with Travoprost Eye Drops,and the observation group was treated with YJBYHWD on the basis of the control group.The clinical effects of the two groups and the changes of TCM syndrome integral before and after the treatment were compared.The changes of visual function indexes(including vision,intraocular pressure,visual field defect range,field photosensitivity),central retinal artery blood flow including systolic peak velocity(SPV),end diastolic velocity(EDV),resistance index(RI)before and after the treatment were measured.Results:The effective rate was 88.57%in the observation group and 68.57%in the control group,and the difference was statistically significant(P<0.05).The TCM syndrome score of either of the two groups was lower than that of the same group before the treatment,and that of the observation group after the treatment was lower than that of the control group,the difference was statistically significant(P<0.05).After the treatment,the vision and visual field photosensitivity of the two groups were higher than those of the same group before the treatment;the intraocular pressure and visual field defect range were lower than those of the same group before the treatment;the visual acuity and visual field photosensitivity of the observation group were higher than those of the control group after the treatment;the intraocular pressure and visual field defect range were lower than that of the control group after the treatment,and all the differences were statistically significant(P<0.05).After the treatment,SPV and EDV of the two groups were higher than those of the sam
