针刺联合金沙排石汤治疗肾结石38例

摘　　要：目的:观察针刺联合金沙排石汤治疗肾结石的临床疗效。方法:将76例肾结石患者按照随机数字表法分为研究组和对照组,每组各38例。对照组接受常规体外冲击波碎石术治疗,研究组在对照组的基础上给予针刺联合金沙排石汤治疗。检测两组患者治疗前后血清中血肌酐(serum creatinine,SCr)、尿素氮(blood urea nitrogen,BUN)及胱抑素C(cystatin C,Cys-C)含量。比较两组患者的临床疗效、尿液代谢情况及治疗前后24 h尿生化指标变化情况。结果:对照组有效率为84.21%,研究组有效率为94.74%,两组患者有效率比较,差异具有统计学意义(P<0.05)。研究组治疗后SCr、BUN、Cys-C含量低于本组治疗前,且低于对照组治疗后,差异具有统计学意义(P<0.05)。两组患者治疗后24 h尿生化指标低于本组治疗前,且研究组治疗后低于对照组治疗后;两组患者治疗后尿量多于本组治疗前,且研究组治疗后多于对照组治疗后,差异均有统计学意义(P<0.05)。研究组高尿钙症、高尿酸尿症、高草酸尿症、低镁尿症及低尿量发生率低于对照组,差异有统计学意义(P<0.05)。两组患者头晕、恶心、腹泻、皮肤瘙痒等不良反应发生率比较,差异无统计学意义(P>0.05)。结论:针刺联合金沙排石汤治疗肾结石患者,可改善患者肾功能及24 h尿生化指标,同时能促进尿液代谢平衡。Objective:To observe the clinical curative effect of acupuncture in combination with Jinsha Paishi Decoction(金沙排石汤,JSPSD) on kidney stone.Methods:A total of 76 patients with kidney stone were divided into the study group and the control group,with 38 cases in each group.The control group received conventional extracorporeal shock wave lithotripsy,while the study group received JSPSD and acupuncture treatment on the basis of the control group.The contents of Serum creatine(SCr),blood urea nitrogen(BUN) and cystatin C(Cys-C) were measured before and after the treatment.The changes of urine metabolism and urine biochemical indexes before and after the treatment were compared between the two groups.Results:The effective rate was 84.21% in the control group and 94.74% in the study group.The difference was statistically significant(P<0.05).The contents of SCr,BUN,Cys-C in the study group were lower than those before the treatment and lower than that after the treatment in the control group,and all the differences were statistically significant(P<0.05).After the treatment,the 24-hour urine biochemical indexes of the study group were lower than those before the treatment,and the urine volume of the study group was lower than that of the control group after the treatment;the urine volume of the two groups after treatment was more than that before the treatment,and the urine volume of the study group was more than that of the control group after the treatment,and all the differences were statistically significant(P<0.05).The incidences of hypercalciuria,hyperuricemia,hyperoxaluria,hypomagniuria and low urine volume in the study group were lower than those in the control group,all of the differences were statistically significant(P<0.05).There were no significant differences in dizziness,nausea,diarrhea,pruritus between the two group(P>0.05).Conclusion:Acupuncture in combination with JSPSD can improve the renal function and 24-hour urine biochemical indexes of the patients with kidney stone,and promote the balance
