从《血证论》探讨肺痿证治

摘　　要：《血证论》共有18次提及肺痿病名及其证治,相关论述散见于阴阳水火气血论篇、脏腑病机论篇及失血诸证篇。其首次描述肺痿肺叶形态翘举萎缩,基本病机为津枯肺燥。津液生成不足或耗伤太过,使肺阴液不足,被火克刑,发为肺痿,与肺治节功能失调密切相关,治疗当生津补肺。血证可导致津液枯竭而引发肺痿,失血和肺痿常互为因果,伴随咳嗽、喘、声音嘶哑等。水虚火旺为肺痿与血证的共同病机,宁血、补血当以补肺为要。肺痿具体治法包括滋补肺阴、培土生金、金水相生、镇敛、宣通等。代表方剂为清燥救肺汤、保和汤等。Lung Paralysis and the treatment of the disease were mentioned in 18 places in TCM classic Xue Zheng Lun,and the related discussions are scattered in the chapters on Yin Yang Shui Huo Qi Xue Lun Pian,Zang Fu Bing Ji Lun Pian and Shi Xue Zhu Zheng Pian. It was described for the first time as that the morphology of the lung lobe was atrophied and the basic pathogenesis was Jin Ku Fei Zao. In other words,insufficient fluid generation or excessive exhaustion causes insufficient Lung Yin and then Lung is punished by Fire and developed into the disease of Lung paralysis,which is closely related to dysfunction of the controlling and regulating power of Lung,and the treatment method should be producing Fluid to tonify Lung. Blood loss can lead to depletion of fluid and cause Lung paralysis,and both of the two are reciprocal causation. The disease is usually accompanied by cough,wheezing,hoarseness. Shuixu Huowang is the common pathogenesis of Lung paralysis and Blood syndrome. The specific treatment methods for lung dysentery include nourishing the Lung Yin,Pei Tu Sheng Jin,Jin Shui Xiang Sheng,Zhen Lian and Xuan Tong.Representative prescriptions are Qingzao Jiufei Decoction and Baohe Decoction.
