黄龙固本汤对2型糖尿病肾病早期微量蛋白尿患者肾功能的影响

摘　　要：目的:观察黄龙固本汤治疗2型糖尿病肾病早期微量蛋白尿患者的临床疗效,并观察其对患者肾功能、血糖及证候积分的影响。方法:80例2型糖尿病早期微量蛋白尿患者随机平均分为观察组和对照组。对照组给予基础治疗,观察组在对照组治疗基础上给予黄龙固本汤口服,疗程为3个月。比较治疗前后患者尿微量白蛋白/肌酐(microalbuminuria/creatinine,ACR)、肌酐(creatinine,Cr)、血尿素氮(blood urea nitrogen,BUN)、糖化血红蛋白(glycosylated hemoglobin,HbA1c)、空腹血糖(fasting plasma glucose,FPG)、餐后血糖(postprandial blood glucose,PBG)水平,中医证候总积分,并比较临床疗效。结果:治疗后两组患者中医证候总积分均下降,但观察组下降幅度明显高于对照组(P<0.01)。观察组和对照组有效率分别为87.5%和67.5%,两组有效率比较,差异有统计学意义(P<0.05)。与治疗前比较,治疗后两组患者ACR、Cr、BUN、HbA1c、FPG、PBG均显著改善,治疗组以上指标改善显著优于对照组,差异均有统计学意义(P<0.05)。结论:黄龙固本汤治疗2型糖尿病肾病早期微量蛋白尿患者临床疗效显著,可显著降低患者中医证候积分、血糖,改善患者肾功能。Objective: To observe the clinical efficacy of Huanglong Guben Decoction in treatment of patients with early-stage type 2 diabetes and early microalbuminuria,and to observe its effects on renal function,blood glucose and syndrome score. Methods:Eighty patients with early type 2 diabetes with microalbuminuria were randomly divided into observation group and control group.The control group was given basic treatment,and the observation group was given Huanglong Guben Decoction orally based on the treatment of the control group for 3 months. Compare urine microalbuminuria/creatinine( ACR),Cr,blood urea nitrogen( BUN),glycated hemoglobin( Hb A1 c),fasting blood glucose( FPG),and Postprandial blood glucose( PBG) levels,total scores of TCM syndromes,and clinical efficacy were compared. Results: The total scores of TCM syndromes of the two groups of patients decreased after treatment,but the decline in the observation group was significantly higher than that in the control group( P < 0. 01). The effective rates of the observation group and the control group were 87. 5% and 67. 5%,respectively. Comparing the effective rates of the two groups,the difference was statistically significant( P < 0. 05). Compared with before treatment,ACR,Cr,BUN,Hb A1 c,FPG,and PBG were significantly improved in the two groups after treatment. The above indicators in the treatment group were significantly better than the control group,and the differences were statistically significant( P < 0. 05). Conclusion: Huanglong Guben Decoction has a significant clinical effect on patients with early-stage type 2 diabetes and early microalbuminuria,which can significantly reduce the TCM syndrome score,blood sugar and improve renal function.
