摘 要:目的探讨阴道微环境和白介素-10(IL-10)水平变化与高危型HPV感染的关系。方法选择2016年1月~2019年1月在舟山医院和甘肃省第二人民医院进行宫颈癌前筛查的患者3879例的资料进行回顾性分析,按照患者是否出现高危型HPV感染进行分组,分为病例组1346例,对照组2533例,通过单因素分析与多因素分析的方式,探讨阴道微环境和IL-10水平变化与高危型HPV感染的关系。结果病例组患者的pH值、乳酸杆菌的量、白细胞级别与对照组比较差异均有统计学意义(t=50.598,χ^2=244.382、27.604,P<0.05),病例组患者的念珠菌检出率高于对照组,差异有统计学意义(χ^2=284.363,P<0.05),两组患者的滴虫检出率数据差异无统计学意义(χ^2=2.802,P>0.05),病例组患者阴道灌洗液中的IL-10浓度高于对照组患者,差异有统计学意义(t=62.260,P<0.05),pH值偏高、乳酸杆菌的量偏少、白细胞偏多、念珠菌阳性、阴道灌洗液中的IL-10浓度偏高,均成为患者出现高危HPV感染的独立危险因素(P<0.05)。结论阴道微环境和IL-10水平变化与高危型HPV感染之间的关系密切,但需要更多研究对各类因素之间的因果关系进行深入分析。Objective To investigate the relationship between changes in vaginal microenvironment and interleukin-10(IL-10)levels and high-risk HPV infection.Methods A retrospective analysis was performed on 3879 patients who underwent cervical cancer screening in Zhoushan Hospital and Gansu Province Second People's Hospital from January 2016 and January 2019.According to whether patients had high-risk HPV infection,they were divided into 1346 cases in the case group and 2533 in the control group.The relationship between vaginal microenvironment and IL-10 levels and high-risk HPV infection was investigated by univariate analysis and multivariate analysis.Results The pH value,the amount of lactobacilli and the white blood cell level of the patients in the case group were different from those in the control group.The differences in the data were statistically significant(t=50.598,χ^2=244.382,27.604,P<0.05).The detection rate of Candidain the case group was higher than that of the control group,and the difference was statistically significant(χ^2=284.363,P<0.05).There was no statistically significant difference in the detection rate of trichomoniasis between the two groups(χ^2=2.802,P>0.05).The concentration of IL-10 in the vaginal lavage fluid of the patients in the case group was higher than that in the control group.The difference was statistically significant(t=62.260,P<0.05).The higher pH,lessamount of lactobacillus,Candida positive,and high concentration of IL-10 in vaginal lavage fluid had become independent risk factors for high-risk HPV infection in patients(P<0.05).Conclusion There is a close relationship between changes in vaginal microenvironment and IL-10 levels and high-risk HPV infection,but more research is needed to conduct an in-depth analysis of the causal relationship between various factors.
