切开复位内固定法和经皮微创锁定加压钢板法治疗胫骨骨折的临床疗效

摘　　要：目的探究切开复位内固定法、经皮微创锁定加压钢板法治疗胫骨骨折的临床疗效。方法以2015年1月至2017年12月本院收治59例胫骨骨折患者,根据患者的临床表现选择34例采取常规切开复位内固定法(切开复位组),其他25例采用经皮微创锁定加压钢板法(微创手术组)。对比两组的临床效果。结果经过12~24个月的随访工作,两组患者术中出血量、术后出血天数及术后膝关节功能比较差异无统计学意义;微创手术组手术切口长度、骨骼愈合速度优于切开复位组,两组比较差异有统计学意义(P<0.05)。结论经皮微创锁定加压钢板法出血量低、创伤小、骨骼愈合状况良好、切口短,临床治疗效果显著优于常规切开复位固定疗法。Objective To compare the clinical effects of open reduction and internal fixation and percutaneous minimally invasive locking compression plate in the treatment of tibial fractures.Methods 59 cases of tibial fracture were treated with internal fixation admitted in our hospital from January 2015 to December 2017 according to the clinical manifestations,34 cases were treated with conventional open reduction and internal fixation(open reduction group)and 25 cases with percutaneous minimally invasive locking compression plate(minimally invasive surgery group).Then the specific clinical effects of two groups were compared.Results After 12-24 months of follow-up,there was no significant difference in the amount of bleeding during operation,the number of bleeding days after operation and the function of knee joint between the two groups.However,in terms of incision length and bone healing speed,the final effect of percutaneous minimally invasive surgery group was better than that open reduction group,and the difference was significant(P<0.05).Conclusion Percutaneous minimally invasive locking compression plate has the advantages of stable fixation,reliable components,low intraoperative bleeding,small wound,good bone healing and short incision.The comprehensive clinical effect is superior to conventional open reduction and fixation.
