摘　　要：目的探讨立体“Z”成形术治疗鼻瓣区膜性狭窄的疗效。方法回顾性分析2012年6月~2018年1月就诊于我科的17例鼻瓣区膜性狭窄患者,对所有患者膜性狭窄部位行立体“Z”成形术,于术前及术后1年对鼻塞进行视觉模拟量表(visual analogue scale,VAS)评分并利用Eccovision鼻声反射仪测量所有病例鼻腔最小横截面积(minimal cross-sectional area,MCA)。术前、术后VAS评分及MCA分别采用Wilcoxon秩和检验和t检验进行分析,P<0.05为统计学有意义的判断标准。结果术后随访1年,17例患者23侧VAS评分由术前的7.01±1.12降低为术后的2.13±0.46(P<0.001)。MCA由术前的(0.23±0.12)cm^2为术后增加为(0.63±0.15)cm^2(P=0.004)。结论立体“Z”成形术可明显改善鼻瓣区膜性环形狭窄患者的鼻塞症状及最小横截面积。OBJECTIVE To investigate the effect of stereoscopic'Z'-plasty on membranous narrowing of nasal valve.METHODS The clinical data of 17 patients with membranous narrowing of nasal valve from June 2012 to Jan 2018 were retrospectively studied.All of the patients received a stereoscopic'Z'-plasty for treatment of membranous narrowing of nasal valve.Ecco vision acoustic rhinometry was used to measure the minimum cross-sectional area(MCA)preoperatively and 1 year after operation.And the visual analogue scale(VAS)was used to evaluate the changes of nasal obstruction.Wilcoxon rank sum test and student't-test were used to analyze the difference.RESULTS MCA was(0.23±0.12)cm^2 preoperatively and(0.63±0.15)cm^2 at 1 year after operation.Significant difference was found in MCA(P=0.004).VAS was 7.01±1.12 preoperatively and 2.13±0.46 at 1 year after operation and the change had statistical significance(P<0.001).CONCLUSION'Z'-plasty is an effective method for treatment of membranous narrowing of nasal valve.
