摘 要:目的提高门诊药学服务水平与质量,并为统一审方标准提供参考。方法依据处方管理相关法律法规、药品说明书、临床诊疗规范及指南,对2018年医院门诊药师审核干预的处方进行回顾性分析。结果药师对超药品说明书用药、重复给药、药物配伍禁忌存在认知误区,导致处方漏审、错审及过度干预事件频发。结论药师应努力加强专业知识学习,科室需从管理层面持续开展处方审核培训,使处方审核真正成为保障医疗安全和提高药学服务质量的重要环节。Objective To improve the level and quality of outpatient pharmaceutical services and provide a reference for a normalized prescription review standard.Methods According to the prescription management related laws and regulations,drug specifications,standard and guidelines of clinical diagnosis and treatment,the prescriptions for the review interventions of the clinic outpatient pharmacists in 2018 were retrospectively analyzed.Results The pharmacists had some misunderstandings in the cases of whether the drug was over-instructed or repeated administrated,whether there was a potentially clinically significant drug interaction or incompatibility in the prescription review interventions,which led to missed review of prescriptions,misjudgment and excessive intervention.Conclusion Pharmacists should enhance their professional knowledge with effort;clinical departments need to normalize prescription review training from the management level,making prescription review truly becomes an important part of ensuring medical safety and improving the quality of pharmaceutical services.
