妊娠期肝内胆汁淤积症产妇胎盘血流灌注及AQP1表达的相关性研究

摘　　要：目的分析妊娠期肝内胆汁淤积症(ICP)与胎盘灌注及胎盘上水通道蛋白1(AQP1)mRNA和蛋白表达的相关性,探讨妊娠期肝内胆汁淤积症导致不良妊娠结局的病理机制.方法术前测量ICP组和正常对照组子宫动脉血流阻力指数(RI)、搏动指数(PI),采用Real-timePCR和Western-blot法检测ICP组和正常对照组产妇的胎盘、胎膜及脐带中的AQP1mRNA和蛋白表达.结果ICP组孕妇子宫螺旋动脉的PI和RI均显著高于正常对照组,差异均有统计学意义(P<0.05).ICP组产妇胎盘、胎膜及脐带中AQP1mRNA和AQP1蛋白的表达均较正常对照组显著升高,差异有统计学意义(P<0.05),且ICP组中胎盘、胎膜中表达量较脐带中表达量明显增加,差异有统计学意义(P<0.05).结论妊娠期肝内胆汁淤积症时胎盘灌注异常,同时胎盘、胎膜及脐带中AQP1的表达也出现代偿性的改变.Objective To investigate the correlation between intrahepatic cholestasis of pregnancy(ICP)and placental perflision and aquaporin 1(AQP1)expression,and to understand the pathological mechanism of intrahepatic cholestasis of pregnancy which lead to adverse pregnancy outcomes.Methods The resistance index(RI)and pulsatility index(PI)of uterine artery were measured before operation in ICP group and normal pregnancy group.The expression of AQP1 in placenta,fetal membranes and umbilical cord of ICP group and normal control group were detected by Real-time PCR and Western-blot.Results The PI and RI of uterine spiral artery-in ICP group were significantly higher than that in control group(P<0.05).The expression of AQP1 in placenta,fetal membranes and umbilical cord of ICP group were significantly higher th?m that of nomial group(P<0.05),while in the ICP group the expression of AQP1 in placenta and fetal membranes of were significantly higher than that in umbilical cord(P<0.05).Conclusions When the placental perfusion is abnormal in intrahepatic cholestasis of pregnancy,it seems that the expression of AQP1 in placenta,fetal membr?mes and umbilical cord appears compensative changes.
