股前外侧游离皮瓣加中厚植皮修复全足脱套伤

摘　　要：目的探讨股前外侧游离皮瓣加中厚皮植皮修复全足脱套伤所致软组织缺损的方法和疗效.方法回顾性分析2012年1月至2018年2月12例采用股前外侧游离皮瓣修复足底加中厚皮植皮修复足背的方法治疗全足脱套伤所致软组织缺损患者的临床资料.结果所有患者均获得随访,随访时间12~36个月(平均18个月).除1例皮瓣远端部分坏死,其余皮瓣全部成活.足背植皮大部分成活,足底皮瓣恢复保护性感觉,患足恢复良好的外观及基本的行走功能.随访期间,未发生需要手术治疗的压疮.结论全足脱套伤是一种严重的软组织损伤,应用股前外侧游离皮瓣加中厚皮植皮的方法修复全足脱套伤所致软组织缺损,术后外观及功能恢复满意.Objective To investigate the methods and efficacy of reconstruction of degloving injury of total foot with anterolateral thigh flap and split thickness skin graft.Methods From Jan.2012 to Feb.2018,12 cases with degloving injury of total foot were treated in our department.The plantar soft tissue defects were reconstructed by anterolateral thigh flaps and the dorsal soft tissue defects were repaired by split thickness skin grafts.Results The patients were followed up for 12 to 36(mean 18)months.All the pl.intar flaps survived with protective sensory recovery except for one distal flap necrosis.The dorsal skin survived mostly.All the feet got good cosmetic and essential functional result without stress lesions needed surgery.Conclusion The degloving injury of total foot is serious soft tissue damage.Reconstruction of degloving injury of total foot with anterolateral thigh flap and split thicknss skin graft can get satisfied cosmetic and functional result.
