玻璃体手术前联合曲安奈德眼内注射治疗脉络膜脱离型视网膜脱离疗效分析

摘　　要：目的观察术前眼内注射曲安奈德(TA)对玻璃体手术治疗脉络膜脱离型视网膜脱离的疗效.方法回顾性分析62例(62眼)脉络膜脱离型视网膜脱离[增殖性玻璃体视网膜病变(PVR)≤C1级]患者行玻璃体手术的临床资料.术前联合TA眼内注射患者为A组31(31眼)例,常规术中应用TA患者为B组31(31眼)例,比较两组不同PVR分级的手术效果进行分析.结果两组术后最佳矫正视力(BCVA)均较术前明显改善(P<0.05),A组术后BCVA、复发性视网膜脱离及PVR控制情况均显著优于B组(P<0.05),且A组并发症发生率更低(P<0.05).结论术前眼内注射TA进行预手术处理,能够提高玻璃体手术治疗无明显PVR的脉络膜脱离型视网膜脱离的疗效,值得临床推广应用.Objective To observe the therapeutic etFects of intraocular injecrion of triamcinolone acetonide(TA)before vitrectomy for retinal detachment associated with choroidal detachment.Methods Data of 62 patients(62 vitrectomized eyes)with retinal detachment associated with choroidal detachment with proliferative vitreoretinopathy(PVR)less than grade Cl were reviewed.31 patients(31 eyes)in group A were treated with intraocular injection of TA before vitrectomy.In group B(31 patients,31 eyes),TA was applied conventionally in vitrectomy.Clinical outcomes of different PVR grade in the two groups were compared and analyzed.Results Postoperative best corrected visual acuity(BCVA)of two groups was all significantly improved(P<0.05).Patients in group A had significantly better postoperative BCVA and control of recurrent retinal detachment than group B(P<0.05).Meanwhile,the ratio of PVR development and surgical complications in group A were obviously lower than group B(P<0.05).Conclusion Intmocular injection of TA before vitrectomy for retinal detachment associated with choroidal detachment with PVR less than grade C1 improves the clinical effects,and deserves further application and promotion.
