水化疗法应用于气管切开非机械通气患者肺部感染中的疗效观察

摘  要:目的探讨水化疗法应用于气管切开非机械通气患者肺部感染的疗效.方法气管切开非机械通气患者80例,按随机数字表法分为对照组与观察组,每组各40例,对照组予常规预防肺部感染,观察组在此基础上增加水化疗法.在治疗前及治疗2周后进行痰液粘稠度判定、肺部感染评分、血常规中的炎性因子和降钙素原数值比较.结果治疗后,观察组和对照组稀释痰液的有效率分别为100%和20%;两组患者肺部感染评分比较,差异有统计学意义(P<0.05);观察组白细胞计数、中性粒细胞计数、超敏C反应蛋白、降钙素原[(7.57±2.73)、(5.41±2.53)、(11.63±8.67)、(0.17±0.11)]优于治疗前[(9.08±3.18)、(7.07±2.77)、(23.40±20.56)、(0.25±0.19)]和对照组治疗后[(9.44±2.07)、(6.91±1.87)、(25.69±15.30)、(0.27±0.15)],差异均有统计学意义(P<0.05).结论水化疗法与常规预防肺部感染的措施相结合可显著降低气管切开非机械通气患者的肺部感染的发生率.Objective To investigate the effect of hydration therapy on lung infections in post-tracheotomy patients without mechanical ventilation.Methods Eighty post-tracheotomy patients without mechanical ventilation were divided into test group and control group according to the random number table method,with 40 cases in each group.Conventional therapies for preventing lung infections were given,and hydration therapy was added on the basis of it in the treatment group.Before treatment and after 2 weeks of treatment,the consistency of sputum viscosity,lung infection scores,and inflammatory factors and calcitonin levels in blood routine were compared between 2 groups.Results After treatment,the effective rates of dilute sputum were 100%in the test group and 20%in the control group respectively;the change of lung infection scores was significant in the hydration therapy group(P=0.000),and no significant in the controls(P=0.134);the scores of lung infections between two groups were statistically different(P<0.05).The leukocyte count,neutrophil count,hypersensitivity C-reactive protein,calcitonin value[(7.57±2.73),(5.41±2.53),(11.63±8.67),(0.17±0.11)]in the treatment group,were better than their baseline levels[(9.08±3.18),(7.07±2.77),(23.40±20.56),(0.25±0.19)]and the control group[(9.44±2.07),(6.91±1.87),(25.69±15.30),(0.27±0.15)]with statistical significance(P<0.05).Conclusion Hydration therapy combined with conventional methods to prevent lung infection can significantly improve lung infection in post-tracheotomy patients without mechanical ventilation and reduce the incidence of pulmonary infection.
