摘  要:目的探讨CD19^+EBI3^+p35^+B细胞(IL35+B细胞)在系统性红斑狼疮(SLE)患者外周血中的表达及临床意义.方法选取活动期SLE患者16例、稳定期SLE患者21例及健康对照者17例.运用流式细胞术检测外周血CD19^+EBi3^+p35^+B细胞表达频率并统计分析,同时分析IL35+B细胞与SLEDAI评分和C3、C4的相关性.结果(1)IL35+B细胞活动期SLE为(1.063±1.467)%,稳定期SLE为(3.458±3.124)%,健康对照为(4.639±3.047)%.活动期SLE患者IL35+B细胞表达频率低于稳定期SLE与健康对照组(P<0.01).(2)SLE患者IL35+B细胞与SLEDAI评分呈负相关,而与C3、C4呈正相关(P<0.01).结论活动期IL35+B细胞表达明显降低,且随着SLEDAI评分的升高而下降,并与C3、C4呈正相关.提示IL35+B细胞下降导致负性免疫调节机制的紊乱可能与SLE的发病机制相关.Objective To investigate the expression and clinical significance of CD19^+EBi3^+p35^+i35B cells(IL35B cells)in peripheral blood of parients with systemic lupus erythematosus(SLE).Methods 16 active SLE patients,21 stable SLE patients and 17 healthy controls were selected as subjects.Flow cytometry was used to detect the expression frequency of CD19^+EBi3^+p35^+i35B B cells in peripheral blood and to analyze the correlation between IL35+B cells and SLEDAI score,C3 and C4.Results(1)The SLE of IL35^+ B cells in active phase was(1.063±1.467),(3.458±3.124)in stable phase and(4.639±3.047)in healthy control.The expression frequency of IL 35^+ B cells in active SLE patients was lower than that in stable SLE and healthy control group(P<0.01).(2)IL35 B cells in SLE parients were negatively correlated with SLEDAI score,but positively correlated with C3 and C4(all P<0.01).Conclusion The expression of IL35 B cells in active phase decreases significantly,and decreases with the increase of SLEDAI score,and is positively correlated with C3 and C4.These results suggest that the disorder of negative immune regulation caused by the decrease of IL35+B cells may be related to the pathogenesis of SLE.
