复方霜桑叶合剂联合中医体质干预治疗尘肺病疗效观察

摘　　要：目的观察复方霜桑叶合剂联合中医体质干预治疗尘肺病的临床疗效及安全性。方法选取山东省职业卫生与职业病防治研究院、济南医院、莱芜钢铁集团有限公司医院诊治的尘肺稳定期住院患者107例,按分层随机数字表法分为治疗组与对照组,治疗组53例包括气虚质36例、阳虚质17例,对照组54例包括气虚质40例、阳虚质14例。治疗组采用常规西药联合复方霜桑叶合剂及体质干预药物(阳虚质加用右归丸,气虚质加用六君子汤)治疗,对照组采用常规西药联合复方霜桑叶合剂治疗,2组疗程均为3个月,分别对治疗前后肺功能、圣乔治呼吸问卷(SGRQ)评分、CAT量表评分、生活质量评估问卷(mMRC)量表评分、6 min步行实验、不良反应进行观察,评价治疗效果及安全性。结果2组患者治疗后SGRQ、mMRC、CAT评分均较治疗前降低,治疗组较对照组下降更明显,差异均有统计学意义(P均<0.05);治疗组患者治疗后FEV1、FEV1/FVC、6 min步行实验距离较治疗前及对照组治疗后明显升高,差异均有统计学意义(P均<0.05)。结论在复方霜桑叶合剂治疗尘肺病基础上,联合中医体质干预能明显改善患者的肺功能,提高患者运动耐力,改善患者的生活质量。Objective It is to observe the clinical efficacy and safety of compound cream mulberry leaf mixture combined with traditional Chinese medicine constitutional intervention in the treatment of pneumoconiosis.Methods 107 cases of pneumoconiosis patients at stable stage in Shandong Provincial Occupational Disease Hospital,Jinan Hospital,and Laigang Hospital from July 2016 to January 2017 were divided into treatment group and control group according to the stratified random number table method.There were 53 cases in the treatment group,including 36 cases of Qi deficiency,17 cases of Yang deficiency,and 54 cases in the control group,including 40 cases of Qi deficiency,and 14 cases of Yang deficiency.The treatment group was treated with conventional western medicine combined with compound cream mulberry leaf mixture and physical intervention drugs(Yang deficiency treated with Yougui Pill,Qi deficiency with Liujunzi Decoction),the control group was treated with conventional western medicine combined with compound cream and mulberry leaf mixture.The two groups were treated for 3 months.The lung function,the Saint George Respiratory Questionnaire(SGRQ),quality of life assessment questionnaire(mMRC),CAT score,six-minute walking test,and adverse reactions were observed to evaluate the treatment effect and safety in both groups.Results After treatment,the SGRQ score,CAT scale score,and mMRC scores were lower than those before treatment in both groups,and the treatment group was significantly lower than the control group,the differences were all significant(P<0.05).The length of 6-minute walk test,FEV1,FEV1/FVC were significantly increased after treatment in the treatment group compared with that before treatment and in the control group,the differences were all significant(P<0.05).Conclusion combination with traditional Chinese medicine physical intervention can significantly improve pneumoconiosis patients’lung function,improve exercise endurance,and improve patients’quality of life on the basis of compound cream mulberr
