采用椎弓根钉棒系统治疗骨盆骨折的临床应用研究

摘　　要：目的分析采用椎弓根钉棒系统治疗骨盆骨折的临床效果。方法选择该院2013年1月至2018年1月收治的TileB、C型骨盆骨折患者80例作为研究对象,随机分为对照组和观察组。对照组采用传统外固定架固定手术治疗,观察组采用椎弓根钉棒系统经小切口皮下固定治疗,比较两组临床效果。结果观察组手术时间、住院时间、骨折愈合时间均小于对照组,差异有统计学意义(P<0.05)。观察组优良率为90.0%(36/40),高于对照组的77.5%(31/40),差异有统计学意义(P<0.05)。术后功能恢复情况,观察组就坐(8.99±0.67)分、疼痛(27.51±2.59)分、工作(32.02±3.84)分、站立(17.95±1.62)分均高于对照组就坐(7.52±0.77)分、疼痛(20.03±2.64)分、工作(28.64±4.02)分、站立(13.65±1.42)分,差异有统计学意义(P<0.05)。对照组并发症发生率20.0%(8/40),明显高于观察组的5.0%(2/40),差异有统计学意义(P<0.05)。结论针对骨盆骨折患者采用椎弓根钉棒系统经小切口皮下固定治疗可缩短手术时间及住院时间,促进骨折愈合,有效实现骨盆功能的恢复,降低术后并发症发生率,值得临床推广。Objective To analyze the clinical effect of adopting the pedicle screw system in the treatment of pelvic fracture.Methods Eighty cases of Tile B and C type pelvic fractures in this hospital from January 2013 to January 2018 served as the study subjects and randomly divided into the control group and observation group.The control group adopted the fixation operation by adopting the traditional external fixed frame,while the observation group adopted pedicle screw and rod system through small incision subcutaneous fixation.Then the clinical effects were compared between the two groups.Results The operation time,hospitalization stay and fracture healing time in the observation group all were smaller than those in the control group,and the difference was statistically significant(P<0.05).The excellent and good rate in the observation group was 90.0%(36/40),which was higher than 77.5%(31/40)in the control group,and the difference was statistically significant(P<0.05).The sitting(8.99±0.67)points,pain(27.51±2.59)points,work(32.02±3.84)points and standing(17.95±1.62)points in the observation group were higher than(7.52±0.77)points,(20.03±2.64)points,(28.64±4.02)points and standing(13.65±1.42)points in the control group,and the differences were statistically significant(P<0.05).The incidence rate of complications in the control group was 20.0%(8/40),which was significantly higher than 5.0%(2/40)in the observation group,and the difference was statistically significant(P<0.05).Conclusion Adopting the pedicle screw and rod system treatment through small incision subcutaneous fixation in the patients with pelvic fracture can shorten the operation time and hospital stay,promotes fracture healing,effectively achieves the recovery of pelvic function,reduces the incidence of postoperative complications and is worth popularizing in clinic.
