摘　　要：目的本研究旨在探讨PPAR-γ激活剂吡格列酮对糖尿病大鼠肾缺血再灌注损伤(ischemiareperfusion injury,IRI)的影响。方法糖尿病大鼠模型采用腹腔注射STZ法建立。通过双侧肾动脉夹闭45 min后恢复血流灌注建立糖尿病大鼠肾IRI模型,实验组糖尿病大鼠术前一周给予吡格列酮(每日10 mg/kg)。术后24 h检测肾功能(血清肌酐和尿素氮)、肾组织病理学、肾组织bcl-2、bax以及caspase-3、caspase-8等蛋白的表达,并且通过TUNEL法检测肾细胞凋亡情况。结果吡格列酮治疗可以明显降低糖尿病大鼠肾IRI后血清肌酐和尿素氮水平,减轻肾组织中肾细胞的损伤和肾细胞的凋亡,可以升高肾组织中bcl-2蛋白表达,降低bax蛋白表达,而且吡格列酮治疗可以抑制肾组织中caspase-3和caspase-8蛋白剪切片段的表达。结论吡格列酮可以通过抑制肾细胞的凋亡保护糖尿病大鼠肾IRI。Objective To investigate the effect of pioglitazone,a PPAR-γactivating agent,on renal ischemia-reperfusion injury(IRI)in diabetic rats.Methods Diabetic rat model was constructed by intraperitoneal injection of STZ.The renal IRI model of diabetic rats was established after bilateral renal artery occlusion for 45 minutes.The diabetic rats in the experimental group were given pioglitazone(10 mg/kg per day)for one week before surgery.Renal function,including serum creatinine and urea nitrogen,renal histopathology,expression of bcl-2,bax,caspase-3,caspase-8 and other proteins in renal tissues were detected at 24 h after surgery.TUNEL assay was used to detect the renal cell apoptosis.Results Pioglitazone significantly decreased the level of serum creatinine and urea nitrogen,reduced the damage and apoptosis of renal cells,increased the expression of bcl-2 and decreased the expression of bax in renal tissues.Furthermore,pioglitazone inhibited the expression of caspase-3 and caspase-8 protein fragments in renal tissues.Conclusions Pioglitazone could protect diabetic rats from renal IRI by inhibiting the apoptosis of renal cells.
